Talk to me about your DH's SCSA DNA frag test

ladies of hottness -

We got our results back from Mister D's SCSA DNA frag test. Things are not looking super good.

DFI = 46.9% (fair to poor = 25% - 50%, very poor = more than 50%)

HDS = 12.9% (seemingly fair. greater than 25% = poor)

"The current established clinical threshold is 25% DFI for placing a man into a statistical probability of the following: (a) longer time to natural pregnancy, (b) low odds of IUI pregnancy, (c) more miscarriages, or (d) no pregnancy. Changes in lifestyle as well as medical intervention can lower the %DFI to increase the probability of natural pregnancy. Couples of men with >25% DFI are counseled to try ICSI and when in the >50% range may consider TESE/ICSI. The SCSA(®) simultaneously determines the % of sperm with high DNA stainability (%HDS) related to retained nuclear histones consistent with immature sperm; high HDS values are predictive of pregnancy failure."

Also I've read that these #'s are more difficult to change with diet, supplements etc because they have more to do less alterable things like age.

We've already been using ICSI and would continue that plan.

I know what the numbers mean but I'm wondering if anyone else's partner had similar numbers, was there any change in a plan?

If we cycle at CCRM, I know they have more intervening methods than ICSI, to try and get the very best sperm.

IVF #1 April 2012 = BFN, IVF #2 June 2012 = BFP. U/S 7/23 = saw heartbeat but measuring behind. Follow up U/S on 7/30 - no heartbeat. D&C 8/2. Trisomy 12. IVF #3 Oct 2012 = Chemical Pregnancy

Phone consult with CCRM on 12/12/12 - ODWU 1/4/13 - both tubes clear(!) - AFC 24, AMH 3.2, FSH 9.6, LH 5.4, E2 25. DH has high frag rate but improved!
IVF #4 March 2013 CCRM. EP protocol w/ Menopur, Gonal-F & Dexamethasone. ER 3/29 & IMSI, PICSI. 43R 13M 10F 6blasts bio'd. CCS testing reveals 3 normals!!! FET 5/31/13

We have severe MFI, but never really dug too far into his side of things because my DOR caused us to go right to DEmbryos. I wish I knew more about it for you, but just wanted to say that I'm sorry you didn't get good news and I hope you find a plan that works well for you.

12/2/11 - IUI #1- BFN
8/1/12 - IVF #1 - Zero response from max stims (600iu intramuscularly)
My ovaries seem to be just for decoration

12/6/12 - Adopted five embryos that had been frozen for over ten years!
2/11/13 - Thawed four, sadly two didn't survive. Transferred two beautiful blasts.

2/16/13 - First BFP of my life!
3/27/13 - No heartbeat at 9w0d. D&C the following day.

4/23/13 - Matched to adopt five new embryos!

I am really sorry to read this. ICSI won't fix anything unfortunately. It is like looking for a needle in a haystack and hoping you pick a "good" sperm. It can happen of course and the more eggs you have to try, the better your chances of the embryologist picking a good sperm. It is a numbers game: how many times can you endure IVF waiting for a good sperm to be IcSI'd with the egg? Obviously that answer is different for everyone. There is a procedure called PICSI that can help. It is a special medium that is "like" an eggshell and the best sperm are attracted to it. The embryologist would pick those sperm to inject. With regular IcSI the embryologist cannot see which sperm are the most highly fragmented and that is why it is so hard. 

 One of the girls on the FB board just got two AA blasts despite her DH's high frag %. She is still in the 2WW but we are hopeful for her that a good one was found(she did use donor eggs though as hers were questionable. 

Vitamins may help reduce the frag.  Get him on them ASAP. I was looking at proceptin(google it, it is expensive though but a girl on the DE checkin said it helped her DH's frag level immensely. A cocktail I was told was Vit c 500mg/ 2x day, Vit E 400mg 2x per day, proxceed(very pricey) one lack per day, pycngenol 100mg 2x/day

I have no real advice - but I am glad you got these results before talking to CCRM.  I will be interested to hear what they have to say to you about it.   GL with your consult tomorrow!

CCRM offers PICSI and IMSI.

I'm hopeful that one of these, or both would be useful to us. 

Our results also explain a bit more about our situation and help me to make more sense about what is going on totally. We both have issues.

I'm sorry Mr. D's results were less than stellar, but hope with this new info and possibly cycling at CCRM you will find success! FX tomorrow's consult goes extremely well!!
