Where to seek a second opinion in NYC

Hi, ladies.  I haven't intro'd on here yet; I've refrained because though I've been on the IF board since early this year I took a few-month break after the failure of my first IVF cycle and have only been back for a couple of months.  But I've been with you quite a while in spirit and will "officially" intro when I've been back a little longer.

In the meantime I was hoping the East Coasters among you could help me with a question.  As you see from my siggy, I've got a fairly rare issue where my IVFs yield almost no mature eggs -- even though everything looks fine up to retrieval (stimming fine, not a bad number of follies/eggs, normal estrogen for the number of follies).  I love my RE -- I'm at NYU -- but before I go forward with another IVF I think it would probably be wise to seek a second opinion to make sure I'm not missing out on any brilliant ideas for dealing with my situation.

So my question is this:  Where else would you go if you were me?  I know CCRM is great but I have a new job and being gone for 10 days anytime soon just isn't going to fly.  So I'd prefer to talk to other REs in the NY area who have reputations for being problem-solvers.  I know Cornell is quite well-respected, but are they too big to want to think outside the box for someone like me?  I'm also intrigued by SIRM because Dr. S.her in Vegas has written several blog and message board posts discussing issues similar to mine but I don't know anything about his NY office.

I'd be grateful for any suggestions or comments any of you might have. 


Me: 39. DH: 41. TTC since 2009.
FSH: 9.5 at highest. AMH (11/2011): 0.9. AFC: 10ish. HSG: Normal. SA: Normal.
July 2011 - Feb. 2012: 2 Clomid/TI cycles; 3 injectables/IUI cycles. Good response but BFNs.
Dec. 2011: Laparoscopy (clear) + hysteroscopy to remove uterine polyp.

April 2012: IVF #1: FSH 9.2, AFC 7-8. Antagonist protocol.
Started stims (300iu Gonal-F, 150iu Menopur) 3/28.
Added ganirelix 4/1. Trigger 4/5 (7 follies). ER 4/7: 7R, 0M, 0F. :-(

Nov. 2012: IVF #2: Long Lupron protocol.
Started Lupron 10/27. AFC 10+. Started stims (300iu Follistim, 150iu Menopur) 11/8.
Trigger 11/18 (7 follies). ER 11/20: 9R, 1M, 1F. 3dt of my one 6-cell embie on 11/23: BFN.

Dec. 2012: Hail Mary IUI cycle with leftover IVF meds: BFN.

Feb. 2013: IVF #3: Estrogen priming protocol.
Pre-AF: estro patch + ganirelix. AFC 8-10. Started stims (450 Follistim, 150 Menopur) 1/28.
Added ganirelix 2/4. Combined Lupron/HCG trigger 2/10 (6-ish follies).
ET 2/12: 6R, 1M, 1F w/rescue ICSI. My embie didn't make it. :-(

July 2013: DE IVF #1

#1 run for you life away from Dr.Sher in Las Vegas, I had the worst cycle of my life there.

#2 I went to Dr. Tortoriello in NY he did a superb job with my IVF however any of the Sher institutes I have come to find out too late do not have as up to date info on immune issues so if you run into an immune issue I would go with Beer immunology in CA as they can take care of you long distance. 

#3 I went to Dr. Braverman in NY and I would not go to him because he acted like a complete jerk and his staff is awful.

I haven't cycled there, but Dr. Miller at North Hudson IVF in Englewood Cliffs NJ has a reputation for working with exceptional challenges (not saying that's what you are, just that it's her reputation!) so they might be more receptive to out-of-the-box options.

Also i think it would be worth you having a phone consult with CCRM to see what they say as they are the top in the country.

I don't think it ever hurts to get a second opinion.  We did that as well after two failed IVFs.  Then after two more we got a 3rd, 4th, 5th and 6th opinion.    I have talked with CC.RM, C.ornell and S.IRM-NYC.     My case is different than yours - but I got a really different opinion from C.ornell than I did the others [the only change he suggested in my case was to trigger me one day earlier].   I was very doubtful of that actually being our problem - so we decided to not go that route.  We are going to go with S.IRM and working with Dr. T.

If you can't get to CCRM, I would think that Dr Da.vis at Cornell might be up for the challenge. This may intrigue him to figure it out.

Hi!  I see Dr. Traub.  Here is the website http://ivfny.org/ He is the best.  He has the nicest staff and I would reccommend them to anyone.  He is in Staten Island and South Plainfield, NJ (about 30 minutes from Staten Island).  Good Luck!

IVF 12-21-2012 - 2 I think perfect no matter what the dr graded them (2B)embryos transferred Beta on 12-31-12 Positve - Beta is 21
Beta 2 1-2-13 - 31
Beta 3 1-4-13 - 53
Beta 4 1/9 - 212!
Ultrasound 1/10 - Nothing completely empty uterus Beta 1/14 - 743; Beta 1/17 - 1704 ultrasound shows nothing. Officially a miscarriage. 1/21 Beta 3900 Dr. says it ectopic. Ultrasound shows a miracle heartbeat right in the uterus where it belongs! Dr. says I can be cautiously optomistic!!! Hang in there baby!

based on my experience, these are the ones to go to in NY:

1) Drew Tortoriello at SIRM

2) Jeffrey Braverman. I had personal success with him and I think he is one of the best, if not the best! NYU does not even compare. Cornell will not work with immune or thrombo issues. Braverman consults on Cornel ( dr Davis) cases that need immune/thrombo treatment.

Also dr T is quite laid back with meds, while dr B is aggressive. I chose dr B approach and had a healthy baby, while with dr T i had a m/c.
