Can we discuss Assisted Hatching?

I received my IVF consent/info packet on Friday.  My clinic only does ICSI if you "qualify" due to SA results.  However, my packet came with info on AH, which seems to be elective.

 

If your clinic offers elective AH, are you opting in or out?  What are your reasons?  I have my class soon and I will ask questions then, but just wanted to see what you ladies thought!         


Me: 28; DH: 35 1/2008 -- Started TTC
3/2009 -- FD discovered at 17 weeks
1/2011 -- Lap and HSG reveal Stage IV Endo and both tubes blocked.
4/2012 -- Consult with RE for IVF#1 -- All bloodwork and SA are normal -- Repeat HSG - SURPRISE! Both tubes are open.
May, June, August 2012 -- IUI#1-3 -- Letrozole + Bravelle + Ovidrel trigger = BFN
9/2012 -- Lap#2 to remove endo and Hysteroscopy

IVF#1 (Long Lupron) -- 12/6 ER - 20R 11F * 12/11 ET - Transferred 2 excellent blasts *
Beta #1 79.7 * Beta #2 240 * It's a GIRL!!

We did not do it.  There is a small risk of damage to the embryos and increased rate of identical twinning; we also felt that we wanted to let things happen as naturally as possible.  (My RE has similar reasons for not doing ICSI in patients without MFI).  I am under 35 and have no actual IF diagnosis, so I am optimistic that things will do what they need to do.  If I had repeated implantation failures with good quality embryos, I would consider it in the future.

All 6 of our embies were ICSI'd and we have no sperm issues. I was told when we eventually thaw our 3 frozen embryos those will have assisted hatching due to the fact that it seems to give them a better chance of being where they should when they thaw. It really seems like it's up to each clinic and in the opinion of the REs what's offered and what's advised.

We opted in but the RE will only do it if it's completely necessary. Our thinking behind it was if we didn't have as many good looking embryos for whatever reason and the only good looking one(s) needed help with the hatching. 

I totally see it both ways, but I'm kind of in the camp of we can use whatever help we can get at this point, and if that means assisted hatching, I'm ok with it if it's 100% necessary - which we explained to the RE. 

ETA: We're also doing ICSI on all embryos due to MFI.

We didn't do it for our first two transfers (fresh and frozen) but I've asked my RE to call me about pursuing it for our upcoming FET. I don't really know much about it, so I'm hoping my RE can explain the process and if we're the right candidates for it, but given the fact that we've had two implantation "failures" with great looking embryo's, I'm willing to throw everything I can into our next cycle.

TTC #1 since Oct 2009, Dx: Unexplained Infertility
HSG: Right tube all clear, left tube inconclusive
3 failed Clomid Cylces
4/5 lap revealed blocked left tube, but right tube is open
April/May- 50 mg Clomid + Ovidrel + IUI #1= CANCELLED left ovary ovulating
June 1- 100 mg Clomid + Ovidrel + IUI #1 = BFN
June 28- 100 mg Clomid + Ovidrel + IUI #2 =BFP!
7/12 Beta #1= 14 Beta #2= >5 Chemical Pregnancy :(
July-Nov long break to recover emotionally and financially
Nov 24- 100 mg Clomid + Ovidrel + IUI #3= BFN
Jan 22- Gonal-F + Ovidrel + IUI# 4 = CANCELLED no response on right

TTC since 2006 3 Fertility Clinics Test, Pills & Injectables....oh my. Numerous failed IUI's 7 or 8 or more? IVF #1 June 2012 ~ praying this is the only one ER 6/13/12 - 13 ret, 7 fertilized, 7 progressing, ET 6/15/12, BFN & No freeze babies. Onto IVF #2 in October 2012. No, wait, body not cooperating. IVF #2 in November 2012 IVF #2 Began Provera in Sept to make me start. Never started. I O'd on my own??? 2 weeks of BCPs. Period. Baseline & Labs 10/25 Stims started 10/26 ~ 225 Follistim in the am. 150 Bravelle & 5 Lupron in pm. Trigger 11/4, ER 11/6, ET 11/9 - 2 great embies. 3 Freezebabies. Beta 11/23/2012 BFN. Now what??? I don't know if I can keep going? Hey!  I am doing great.  Finally getting used to the side effects of bcps.  Canyou believe next week we will have our classes and finally get this IVFshow on the road?!

Thank you ladies for all of the responses.  My husband I only have one shot at this IVF roller coaster (possibly a FET if there are frosties), so I just want to make sure we make the best decisions possible for us.  I know many ladies in this forum have had multiple fresh cycles and must think I am crazy to think the first try will work, but I have to hold on to hope as its all I have!  :-)

I am also finally getting used to the side effects also of bpcs. It's crazy that by the middle to end of next week we both probably will be starting on injects.

Hope a PAIF response is okay...

We did ICSI with AH.

We have multi-factorial IF dx. I'm 34. Come to find out, when we were in the process, we learned that I had egg quality issues that would have made natural fertilization difficult for the sperm, so ICSI turned out to be quite the blessing for us! IVF was the only way for us to go with my bilaterally obstructed tubes from endo, and Hubby's retrograde ejaculation from bladder and prostate surgeries. 

Now, the AH for us just seemed to be one more step that would get us closer to our goal. When we'd tried for so many years, and tried so many IUIs without success (prior to the tubes being blocked! or knowledge thereof)...it just seemed like we'd try ANYTHING to get us one step closer to our goal. It was our understanding that damage to the embryos is a very minimal risk as it is very uncommon, and that AH actually increases your chances of overall IVF success. That's how we understood it.

We filled out a bunch of consent forms and when we got to assisted hatching, the nurse said they wouldn't use it for us because of my age, but their consent was worded so that if it was used if medically necessary, not an absolute decision. We consented, but they didn't perform it.

We did IVF this summer and AH was offered, but like others said, due to our age it was not needed.    Jewels- I have been PMing you but not sure you are getting them! :)

I am guessing this will be the case for me as well as far as age goes.  I hope the consent is the same way at my clinic.  If it needs to be done, I want it done.  If I can save the $600 and still have a high success rate, then by all means let's save some money.

We may be trying it this cycle because we have been through so many failed cycles. Despite the fact that I am only 31, we are not sure how much egg quality is playing in all of this. I look at it as a little more help
