Low antral count + abnormal postcoital test

Hi everyone, I am new here and I am desperate for help!

After 2 years trying to conceive, I decided to go to a RE and the news were not great. I have a low antral count (last test: 6 folicules total) and the postcoital test was abnormal (only 2 sperms were alive).

He gave us the option to try the IUI, however the changes of getting pregnant are only 15% acording to him and if I plan to have a second child, I might not have a change in one or 2 years, since I will probably rin out of follicules.

In this case, he prefers to do an IVF right away and freeze some eggs for future.

However, my husband freaked out with the IVF low rates of succes versus price.

I would like if anyone with same situation, got pregnant through IUI. Or if you know of any literature about this specific situation, I have been exaustively looking for materiais about this combination of problems and didnt find anything...

Welcome! Sorry you have to be here. What is your age? Have you had any more testing, e.g. Hsg, estradiol, FSH, AMH? Personally, based on the info you gave us so far, I would definitely opt for Ivf vs Iui, but if you are not ready, I don't see the harm in trying a few iuis to see if you get lucky. It may also be nice to try an Iui with clomid or injectables so your RE knows how well you respond to stimulation meds. But, with poor sperm and limited eggs, it sure would seem Ivf would give you a better shot. My AFC did vary about 5-6 follicles per cycle, so yours may fluctuate as well. You would certainly want to know your FSH number, so make sure you ask your RE.


Me: 35, DH: 36, ttc: 3+ years, dx: unknown
10/11: after 2 years, saw a RE, FSH 5.4
11/11: BFP! (surprise after thyroid med & normal hsg),
12/11: missed m/c after 7 week u/s,
1/12: D&C,
6/12 IUI #1: clomid, 5 mature follicles, excellent sperm,= BFN,
7/12: IUI #2: clomid, 3 mature follicles, excellent sperm= BFN,
8/12: IUi #3: clomid, 5 follicles, = BFP!, C/P
10/12 IVF #1: FSH 5.4, estradiol AFC: 14, long Lupron 150/bravelle75,5R/5M/4F, all 4 made it to 5dt, 1 blast and 1 8 cell transferred. No frosties. BFN
IVF #2: AFC 21, MD lupron 12/25, 225bravelle/150men, 4R/4M/3F, 5dt of 1 blast BFN
IVF #3: natural start antagon, starting 3/30, ER 4/11, 12R,11M,10F

Hi! Thanks for responding! I am 33 years old, and I dont know my numbers...I never asked :(  , however the doctor said eveything else is normal.

My fear is if I keep trying with no success, the changes of getting pregnancy will keep decreasing when we decide to do an IVF. I am ready for the IVF, but my DH does not want to consider it due to costs...

is there anything else I should ask the doctor that would help me make a better decision??

If your AMH and FSH levels are normal I would definitely try 2-3 IUIs before moving onto IVF.  If that doesn't work maybe your dh will warm up to the idea of IVF.  Good luck!

Me:35, Using frozen donor sperm. 2009-2010 - several non-medicated "at home" frozen donor sperm cycles = BFN
IUIs 1-3 in 2010 = BFN
4/2012: IUI #4 w/ new RE = BFP Missed m/c due to turner syndrome at 9 weeks, discovered at 12 weeks 3 days on 6/21/12. :(
Summer 2012 IUI 5 & 6 = BFN.
Was planning on moving onto IVF but insurance is requiring 3 injectable IUI rounds.
11/12/2012 - IUI #7 75 gonal-f cd3-12 + trigger. = BFP on Thanksgiving!
Beta 1 at 14dpo = 261 Beta 2 at 16dpo = 639 Good NT scan and Negative MaterniT21 test. It's a boy!

Has your husband has a semen analysis done?  Have you had HSG done?  Those are really key in deciding on IUI vs IVF.  A postcoital test is super old school, and I remember a poster here not too long ago saying that her new (and much younger) RE laughed at the fact that the RE she had been seeing previously did one.

Thanks everyone for the answers. Tomorrow I have another appointment to have the antral count done one more time, and I will ask lots of questions this time!

Hi!

I am new here as well.  I don't have any experience with your clinical issues but I wanted to encourage you with your DH.  We knew I would have to do IVF over three years ago and we are now finally on the same page.  We went back and forth for those years between adoption, fostering and IVF. The cost is super intimidating and for my DH the potential of several frozen embryos was NOT appealing at all! For him it was not a future opportunity for more little blessings but rather an obligation to have more kids in the future and maybe he wouldn't want anymore. Anyway, it's been quite the journey but be patient with your DH.  He probably tends to make decisions logically and investing thousands of dollars with low percentage of return is not logical.  Regardless, if it's in your heart to do it you can't deny that dream a chance at becoming reality. Likely, it willjust take longer for his heart to invest in the dream than it does for yours.  

BTW - I have a friend that got pregnant with IUI, she was on clomid for it, he had weak sperm due to a reversal. She got pregnant with triplets the first time. Not sure about her AFC though.
