XP: What to expect at first MFI appointment?

Morning all.  I hope its okay that I'm posting over here as well, but from my lucking it seems like there are a few more dealing with MFI dx than on 3T.

I'm having a little bit of a tough morning and hoping someone will be able to help out a bit.  We have our first MFI specialist appointment tomorrow.  I have been pretty postive and calm so far, but now I'm nervous about what to expect and am big jumble of nerves.  I go from being very excited to terrified every couple of minutes.

If anyone could tell me a bit about your first appointment (tests ran, time table for results, how soon you had a game plan), I would be so thankful.  I am a huge planner and going into this with such little visibility despite my constant googling is really difficult for me, especially when I'm trying to comfort my extremely anxious H.

SA completed 5/29/2012. No sperm found.
11/12: Dx: Congenital Bilateral Absense of the Vas Deferens.
Genetic Testing needed as this is a mutation of Cystic Fibrosis.
IVF #1 with ICSI planned for 2013.
PAIF/SAIF welcome!

I'm sorry about the azoo dx. There's an azoo check in here on IF weekly (although I don't know which day) full of really smart/loving girls.

Which tests has YH had so far? I'd imagine he's going to have a physical exam of the testies (all uros do), then blood work like testosterone/lh/fsh/estrogen, and schedule an U/S of his testies to look for blockages or varicoceles to start.

At our first appointment with the MFI urologist, she put together a plan for testing to find out what is causing the lack of sperm.

It included a physical exam, a blood test, another SA, a transrectal ultrasound. After all of those results came back, they scheduled DH for a testicular biopsy to see if sperm could be found. 

The first appt though was basically outlining all the different possibilities and outcomes and scheduling the tests.

The waiting really is the toughest part. You go to the appt, find out what all the possibilities are and then have to wait for the tests to be run, wait to find out the results and wait to figure out what to do next. In the meantime, my DH and I decided to go right to the worst-case scenario and started talking about what we would do if that occurred (and it did, so I'm glad we had talked about it). Just so we were ready, ya know? It was good to feel like we were on the same page.

CHant I COMPLETELY understand how you are feeling right now. It is so anxious for both him and you! We have severe MFI meaning all his levels are a mess. That being said there is def. hope depending on dx. We are currently in IVF w/ icsi#2. I actually felt relief after we went through testing and saw specialist, I felt like we were not living in the dark anymore, and didn't feel so alone.

1. They will draw blood to test his hormone levels. FSH,testorone,prolactin etc.  He already had insulin, sugar, BP all that checked.

2. Take a urine sample right there, have hubbs prepared to do so just in case. They test to see of there is an infection, sperm some other things I forget.

3. They will discuss history of diseases, surgeries etc

4.  They will want another SA,so will give a script for that.

5. They will do a physical exam, another words dr. will feel husband to see if he feels vericole, both testicles, etc.

6. Based on all the info. collected they may have him do an U/S  on testicles.

If no sperm can be found they will do a biopsy to see if factory is working.

Timeframe was about 2 weeks with everything give or take. We went to the RE right after.

Just the SA.  H was sure there was a problem since the first time we talked about having kids, way before we got married.  He had an injury as a kid where he urinated blood for days but was too scared to tell anyone.  He produces very little semen, maybe a drop or two, so we had big clues from the beginning that something was wrong.  Thanks for the info, I will definitely look for the check-in!

Thank you!  I wish he would talk about the “what-ifs,” but MH won’t go there yet.  He wants to take it step by step, and when my OB (she’s a close family friend) immediately started talking about DS, he freaked out a bit.  He isn’t unwilling to explore it, but he doesn’t want to talk about it yet. 

 

I just read through the first few months of your blog, it’s wonderful!   Wishing you all the best with IVF #1!!

I totally relate to the emotions you're going thru.  There are so many "what if's" and it can be a roller coaster!  Our urologist was wonderful.  At least now I'm super thankful we know exactly what the problem is (husband has absence of the vas - basically means he was born with a vasectomy) so we're preparing for IVF the beginning of 2013.  Of course, I'm still dealing with the "what if there's no sperm after the biopsy" but I try not to go there.  Taking it one step at a time and keeping yourself off Doctor Internet may help.  Best of luck to you guys!!  <3

I'm sorry your H won't talk about the what-ifs. For me, I NEEDED to talk it all out. We talked about DS before our urologist and RE brought it up so that we knew where we stood on it. We started reading pretty much right after the first SA. Not talking about all the avenues would've made me crazy.  I hope you guys don't need to go the route that we have though! :o)

Aw, thanks for the compliment re: my blog. It's been helpful to jot down all this stuff since it's been such a surreal/weird experience.

I think everyone already answered your questions, but I just wanted to say good luck at your appt. We are MFI too, and it really sucks, but hopefully you will get the answers you have been waiting for. It is really good that you are meeting with a specialist first, before jumping into IVF with an RE. It is important that you figure out as much as you can with the sperm first (causes, solutions, ways to improve...). Hopefully it will help make your future plan as successful as possible. Me (33)- unexplained MH (35)- MFI due to Vericocele. Straight to IVF w/ICSI 9/12, Antagonist = BFN c/p, poor quality blasts, tested sperm dna fragmentation. Results all good, yay! New RE for IVF #2- 1/13., Long Lupron. ER on 1/22 -10R, 9M, 9F. Transfer on hold due to overstimming. FET in Feb. 2 frozen blasts. Another BFN & another c/p. RPL testing all negative besides MTHFR gene, vericocle repair surgery 4/12/13-Bilateral Grade 3 Vericocele found & fixed, IVF #3 with PGS 4/13- 11R, 9M, 9F. Transferred 2 normal=BFN, 3 normal on ice Everyone Welcome!!!

MFI is scary, but you guys going to the specialist is a step in the right direction. They are there to help. Ask questions and take notes because its all overwhelming, especially if youve veen mentally preparing yourself for the problem to lie with you and not him.

 Our urologist spoke with us about hubbys history and to rule out any injuries or surgeries. He did a physical exam and found a variococele. He pulled his record and told us about his low testosterone level. Then we cam up with a game plan of clomid plus potential surgery down the line. It was very thorough and he was very patient.  We went in being pretty nervous and left relieved. I hope your appt goes well for you guys. Feel free to PM me if you ever need to talk. MFI is frustrating sometimes, but just know itll all work out
