Talk to me about 3DT.. # of cells, fragmentation, # put back?

Last I heard (yesterday, seems like forever long ago!) was out of the 5 retrieved, 3 fertilized. So... we're going in tomorrow for our 3dt.

What am I looking for? # of cells? What's considered bad bad in terms of fragmentation? We'd already decided to put back 2 when we thought we'd have a bunch of eggs. Since there's only 3, would you put all 3 back if they're all still growing? Or try to freeze one?

Anything else I should know or words of encouragement? Good links to 3dt info?

I'm so nervous they're going to call in the morning and tell me they all arrested. Ahh! I'm freaking out slightly :) LOL

ETA: Fragmentation.. What causes that? Is it an egg thing or a sperm thing? And I know sperm issues can cause embie arrest between day 3 and 5.. but if we always transfer at day 3.. how will we know what they'd do before day 5? Ha. :)

The first day the one cell should become two....the second day the cells should split and become 4 cells.....the 3rd day the four should split and be at least 8. Less than 8 means a slow grower where  8-10 is ideal, and more than 10 is a quick grower (not really ideally a good thing either). However.....not that it worked for me....but there have been women getting pregnant from 4, 5, 6, 7 cells. It doesn't effect the development of the child it just means whether or not it has a good chance of implanting and continuing growth.  Mostly all embryos have some degree of fragmentation....I think I read somewhere 10% is ok and the higher the percent the lower the chance of implantation. It just depends on what you are told tomorrow in terms of growth...ask the RE what he/she suggests as for freezing. While its a common occurrence on these boards for women to have lots to freeze....its apparently not as common for the whole of us so don't be discouraged if there isn't anythign to freeze....although my FX are crossed for you that all 3 are picture perfect for you tomorrow :)

On day 3 your embies should be 6 to 8 cells. Your RE should advise you the number of embryos to transfer tomorrow, as what was previously recommended could change based on your age and/or their quality. While preferable to have little to no fragmentation, my RE said less than 20 was still an okay embie.

FX your embies are doing great and that your transfer is early! so you can stop worrying about the phone ringing Good luck!

Hope u don't mind me responding, Bumping from my phone and cant see siggys.
I don't have much to add to what PPs said regarding cells and frag. But I do want to let you know with my last 2 iVfs, I went into both of my 3dts with 3 embies. Both times we transferred 2 grade B which is 10 to 25 percent frag at my clinic and I got a BFP both times m/c last time was a chromosomal issue and this one so far so good but don't know if one or two stuck. Just to give you some perspective.
FX for you!! Good luck.

Sounds like all the ladies have given you some good input! Just thought I'd add that mine were only 4-cell at transfer time. I was concerned but I also had an early transfer (7:45am) and the embryologist said there was no need to worry, that given a bit more time that day, they likely would have reached at least 6-cells and better to put in the ute anyway..so I am holding onto that! I GL to you!!! Keeping FX for three awesome embies!

TTC since June 2011
DX: DH (30) severe MFI, severely low count & low motility
Me (32): all clear
Appt with Urologist 5/21/12: exam, ultrasound, bloodwork all normal.
Testicular Biopsy with TESE on 6/8/12. good sperm found! (left side only) froze sperm, failed thaw test :(
Orientation for IVF/ICSI on 6/13/12. Waiting for the green light following biopsy results...results show adequate sperm production both sides.
2nd SA 6/18/12: sample is "adequate for ICSI"
Plan: IVF/ICSI July 2012!
ER: 7/26/12. 15 eggs retrieved, all mature.
TESE/TESA/aspiration from epididymis, no motile sperm found :(
froze all eggs, the saga of praying for good sperm continues.
8/3/12: 2nd opinion from MFI uro on biopsy slides. Suspects "partial late maturation arrest."
Plan: more SAs, third biopsy/TESE with frozen back-up either from DH or DS.
SA 8/17/12: Zero sperm
SA 8/23/12: Zero sperm
9/26/12: SPERM FOUND! 15 eggs thawed, 12 survived and were ICSI'd, only 3 fertilized normally. Refrozen as embies and will thaw in Nov. Please survive and grow!
All 3 survived the thaw on 11/15/12!
FET 11/17/12: transferred 2, one 4B, one 4C. Beta 11/30:BFFN
moving on to DS
DIUI#1 2/18/13,50mg Clomid cd 3-7, Ovidrel trigger, Beta 3/4/13: BFN.

Also wanted to add that with both of my transfers I had quite a bit of fragmentation.  The first time I had a 7 an and 8 cell both grade 4 (Scale of 1-5 with 1 the best) in terms of fragmentation.  Had a CP that cycle (think it was due to progesterone, not the embies).  The second time I had an 8 and a 10 cell, again grade 4.  One stuck, and I'm 9 weeks today.  So try not to worry too much about fragmentation.  My RE said the number of cells is the better indicator of whether they will continue to grow and divide... so right around 8 is your target- but many women have 4,5,6,7 cell embies go on to make beautiful babies.

Ultimately, they don't really have a great way to tell at this point which embies will stick around for the long haul.  What they do inside you is so different from what they do in the lab.  Good luck!

TTC Since November 2010
HSG clear, DH SA #1- Slightly low count & motility SA#2-normal
Lap&Dye/Hysterscopy Feb 2012: Endo stage 2
April 2012: Femera 5 mg & IUI = BFN
May 2012: Femera 5 mg & IUI = BFN
June 2012: Femera 5 mg & IUI = BFN

September/October 2012: IVF #2 BFP! Beta 1= 223 Beta 2 = 480 Beta 3 = 11,440

MsGMChick -- Good luck today!  I'm in the same boat (ET in a couple of hours), and have the same worries/questions about quality, given I only have one embie. :-(  I'll keep my fingers crossed that at least one of your embies is a star!  Let us know how the transfer goes.

Me: 39. DH: 41. TTC since 2009.
FSH: 9.5 at highest. AMH (11/2011): 0.9. AFC: 10ish. HSG: Normal. SA: Normal.
July 2011 - Feb. 2012: 2 Clomid/TI cycles; 3 injectables/IUI cycles. Good response but BFNs.
Dec. 2011: Laparoscopy (clear) + hysteroscopy to remove uterine polyp.

April 2012: IVF #1: FSH 9.2, AFC 7-8. Antagonist protocol.
Started stims (300iu Gonal-F, 150iu Menopur) 3/28.
Added ganirelix 4/1. Trigger 4/5 (7 follies). ER 4/7: 7R, 0M, 0F. :-(

Nov. 2012: IVF #2: Long Lupron protocol.
Started Lupron 10/27. AFC 10+. Started stims (300iu Follistim, 150iu Menopur) 11/8.
Trigger 11/18 (7 follies). ER 11/20: 9R, 1M, 1F. 3dt of my one 6-cell embie on 11/23: BFN.

Dec. 2012: Hail Mary IUI cycle with leftover IVF meds: BFN.

Feb. 2013: IVF #3: Estrogen priming protocol.
Pre-AF: estro patch + ganirelix. AFC 8-10. Started stims (450 Follistim, 150 Menopur) 1/28.
Added ganirelix 2/4. Combined Lupron/HCG trigger 2/10 (6-ish follies).
ET 2/12: 6R, 1M, 1F w/rescue ICSI. My embie didn't make it. :-(
