Immune testing, killer cells, antiphospholipid antibodies testing

Anyone done this or talked with their doctor about it?  I have an RE in the family who recommended this now that I'm going through my second m/c, but my RE doesn't really believe it in. My RE said he'd still let me have the testing done since it can't hurt anything. just curious if anyone has had any experience with this.


Me: 33, DH: 32, Started TTC #1 since November '11
Saw RE April 2012 due to irregular cycles, Dx'd PCOS (normal weight)
Clomid 50mg, 100mg, then Femara 7.5, no response after 7 cycles
Testing = normal CD3 bw, normal HSG, v high antral follicle count; good DH SA
IUI #1 on 7/11/12 but poor monitoring and found out after that I never ovulated
New RE; Begin IVF September '12: Lupron, Gonal-F, Ovidrel trigger
14 days of stims, ER 9/30/12 (17 eggs, 12 fert)
ET 10/5/12, single blast transfer, 6 frozen
Home Digi 10/13 (8dp5dt): BFP! Beta 1: 11, Beta 2: 10 = chemical preg :(
FET #1 single embryo transfer: 11/10/12
5dp5dt:BFP! Beta 1- 122; Beta 2- 160; Beta 3- 202; Beta 4- 276
Poor prognosis, U/S inconclusive, 11/27 stopped all meds
12/12/12 - hcg mid 40s, MTX shots; 12/24/12 - hcg same, 2nd MTX shots;
1/23/13, hcg finally at zero. Blood work shows abnormal NK cell activity, but holding off on treatment this cycle
FET #2: 2/9/13, Beta #1, 2/18/13: 265!!, Beta #2, 2/20/13: 570!!, Beta #3 2/22/13: 1445!! Saw gestational sac and yolk sac on US 2/26/13

There is a lady from another group that I'm in who have tested positive for Natural Killer cells and antiphospholipid antibodies. They added baby asprin pre transfer and Lovenox or dexamethasone after they got their BFP.

I am so sorry for your loss.


I had autoimmune and thrombophilia panels done and those tests were part of the workup... Everything was normal in my case but I think it is well worth doing the testing.  Have you had karyotyping done?  That would be something you should also consider (for both you and DH).

Dx = AMA/DOR, hypothyroid, submucosal fibroid, adenomyosis, mild MFI
IUI#1-3 = BFN
IVF #1 (EPP Antagonist); 3R, 2M, 1F; 3dt of one 12-cell embryo = BFN
IVF #2 (EPP MDLF); 6R, 4M, 3F; 3dt of 1-7 cell, 1-8 cell & 1-10 cell = BFP; confirmed C/P @ 4w2d
Genetic carrier screening negative (CF, Fragile X, SMA, Tay Sachs)
Karyotyping, autoimmune & thrombophilia panels all normal
IVF#3 (DE); 13R, 12M, 8F; 5dt of two “excellent quality” blasts = BFP
Beta#1 = 203, Beta #2 = 81, Beta #3 = 17; confirmed C/P @ 5w1d

I had all that stuff done. It came up positive for natural killer cell activity, so I was tested to see if intralipid or IVIG were better for treating that. Both came up as suppressing equally well, so I was on intralipid for all of my Clomid cycles, my one IUI and an IVF cycle. BFNs all around. They switched it up to prednisone for my second IVF, this time an FET, and that was also a BFN. I'm starting to develop my own opinions about whether, even if there is an immune factor to infertility, there is a good treatment available yet. But those are just my uneducated opinions and they're only based on my one case study. Ultimately, it's up to you, your partner and your doctor to decide what's best for you. Good luck whatever you choose!


TTC #1 for about 3.5 years
ME: 37, PCO, immune issues?, teflon uterus (my added diagnosis)
HIM: 38, Perfect. (Showoff.)
Month before starting Clomid, spontaneous pregnancy. m/c at 7 weeks and D&C.
Four months of Clomid, one IUI, with Intralipid each cycle: BFNs all
IVF 1.0 July 2012 -- Lupron, bravelle, menopur, estrogen, progesterone supps, intralipid. 14 ER, 14 fertilized, 1 blast transferred, 2 on ice. BFN
FET 1.1 -- 10/2, transferred our 2 blasts. Lupron, PIO, estrogen, baby aspirin, prednisone. BFN.
IVF 2.0 December 2012 -- Bravelle, Menopur, Ganirelix, Prednisone, Metformin, estrogen, progesterone supps. 22 eggs retrieved on 12/14, 17 mature, 15 fertilized. OHSS anticipated so ET cancelled. 3 embryos frozen.
Broke up with old RE. Moving on to new dude. Maybe the change of scenery will be just the trick for our frosties. On BCPs for possible April FET.

If you're serious about RI workups I'd skip the RE and go directly to an RI. Their testing is much more comprehensive and treatment move acute. We had basic immune/RPL done with an RE and nothing was red flagged, but out RI did extensive testing and found issues; IMHO the treatment was much more focused and complete than what the RE was suggesting. However it's not easy or cheap and some of the treatments are extremely controversial. See the FAQ for more info, but this group is very informative.

I agree with various points by PP.  We did immune dysfunction testing with a RI specialist.  Most RE's do not subscribe to some of the theories.  However, be careful what you wish for, or next thing you know, some over zealous RE / RI will be telling you you can never carry a pregnancy with your DH and you need a surrogate or sperm donor!  I think somewhere in between disbelief and extremism would be great.  

I think karyotyping is a good start for recurrent loss work up, also coagulation work up. Then you can get into the various autoimmune conditions, including thyroid.  Then natural killer cells, HLA typing and so on.  

I agree with PP that there may not be a really effective treatment for these issues yet.  Not to mention, it may not actually be THE issue that is causing you problems.  Still, I think it is probably worth doing.
