Too Many Chocolate Chips (update to 1st RE Appt)

First of all, thanks for all of the good thoughts and prayers today for my 1st RE appt.

It went well, and we have an official diagnosis and a treatment plan....completely overwhelmed by all of the information they gave me today, but it feels good to know that we have a plan!

DH's SA was PERFECT....he can't stop looking at the paper!! 

I had a transvaginal ultrasound (1st one ever, so I was super nervous), but like everyone has told me, it wasn't a big deal.  While doing the ultrasound, the dr. pointed everything out on the screen and explained what everything was.  At one point, she said "See these circles, these are your ovaries.  Now, see all of those chocolate chips on those circles....you have about 30 on each one, and 8 is normal."  At this point she told me that I most likely have PCOS (which I figured, as I have several classic symptoms.....acne and excess body hair).

Her plan is for me to come in next week for my HSG....seeing my anxiety today, she has already prescribed me Valium for the procedure...which already makes me feel better.

The plan she would like to proceed with is Femara CD3-7, ultrasound monitoring appts, trigger shot, and IUI.  I'm on CD1 today, so she said we can go forward with this plan this cycle.  I on the other hand, feel like I need another month.  I am overwhelmed with everything from today, and just want to wait until my next CD1 to begin the actual "intervention" part of this journey.  I'm also considering doing at least one cycle with medication, monitoring, and TI instead of IUI....is it normal for RE's to move straight to IUI instead of letting you try medication with TI first??

I'm glad your app went well and you have a plan :) I don't know about the IUI vs TI, I've seen more people going with TI first, you should do what feels more comfortable to you.

Glad you have a plan! I know what you mean. I actually waited one cycle after they told me what they wanted to do. I needed to mentally prepare. I did TI the first cycle. They said I could do either but that we wanted to get the clomid dose right first we could try one with TI first.

I'm glad you got some answers and a plan. I've seen many start eith TI and others jump right to IUI. There are a lot of things at play with it. Did you get a chance to ask the doctor about starting with TI? If not you could call back snd ask for their reasoning behind recommending that andI why/why not they didn't recommend TI. Even if you do decide to go ahead with IUI you would probably feel better just getting thar answered. Good luck!

my doctor used the exact same comparison when we talking about my TVUS. I don't have typical "too many chips" but my chips are all lined up in a row, which he described as "string of pearls". Love these metaphors!

Glad it went well!  Yay for having a plan.

As for TI vs. IUI - I respond well to Femara (on my 3rd month) and we have a great sperm count, which is why we are doing, and will continue to do, TI.  My RE said that at their hospital, IUI is not done unless you're AMA, have low sperm count, or have other factors that require more aggressive treatment.

What we know right now is that I don't ovulate (or release follicles) on my own, so we're treating that.  If it continues to fail, we'll look into more aggressive insemination techiniques, such as IUI.  I also have PCOS.

Glad you got a plan together. If you aren't ready for IUI, I'm sure if you asked the RE they might be fine w/ you just doing TI. Every RE has a different opinion on the TI vs IUI. REs are in the business of getting you pregnant and some like to go straight to IUI because they think you have a higher sucess rate than TI, even if there isn't an obvious need for it yet.

Glad you had a great RE appointment! I was very overwhelmed as well when I left, but you will soak it all in over the next week or so and get excited to get the ball rolling!

I was shocked that I started an IUI cylce right away with clomid and injects.  I had had all my testing done, but I thought we might just do a couple of rounds of clomid first with TI.   At first I questioned this, but I am trusting the aggressiveness of my RE.  I am 35 and want more than 1 child so after he presented the stats to me and all my options and their probable success rates I relented to do IUI with injects.  Guess that is why he is getting paid the big bucks ( out of pocket bucks in my case!).

Good luck on whatever route you choose to go!

Hey Angie, I know we already spoke about this privately but that is exactly how my RE appt went as well. I'm glad you got some answers dear and good luck with deciding what to do next. As you know I found out in August and have pushed back my hsg and SA until January.. which is quickly approaching. 

I was offered femera with TI first before IUI. I am not sure why some RE's don't suggest that first. Maybe it depends on the severity of your pcos? Obviously had there been an issue with your dh's sa that would have been a no brainer. Anyway, glad you have some answers.

I actually had to ask to do a TI cycle instead of just IUIs (the problem is all me). Of course, they let me, but they reminded me that IUIs have a slightly better sucess rate (since they shoot it all up there). Are you OOP or does your insurance cover anything? For me, the insurance coverage (or lack-there-of) is what helped me decide. All my meds were covered under my prescription coverage but the procedures were barely covered. So, doing a TI cycle would practically be free (only had to pay for Rx copays + couple of u/s)

The cost of the IUI procedure alone with my 50% insurance coverage is $105. I'm not sure what the original rate is before the insurance negotiates the rate. This $105 is about 1/5 the cost of my total IUI cycle (CD3 & CD11 u/s, clomid, gonal-f, ovidrel, progesterone, and beta test). For the added 10% success rate they gave us for IUI (18-25%) vs. TI (5-10%), I think the price we pay for the IUI is more than worth it.

If insurance covers meds and such then it may be worth doing a TI cycle, but otherwise I think IUI is a better value.
