Updated fert report XP

Just talked to my RE and I need some help. We have 3 embryos rated good or fair just what he wanted to end up with. If we had 4 he would have pushed for a 5dt but he's not comfortable and neither am I. I'm worried if it's not a 5dt it won't work. Because of my history he's leaning towards putting all three back but wanted to make sure we are okay with multiples. At this point we would be happy to get a take home baby or babies. I respect and value your opinions so much and would love it if you could weigh in. TIA!

Me 37 MH 40 - TTC since June 2010 - dx with Severe MFI. Straight to IVF with ICSI. IVF #1 - ER 06/13/12 - 9 Eggs Retrieved - 4 ICSI'd - only 2 fert. 06/15/12 - 2DT - 3 cell & 6 cell with fragmentation. Beta 06/29/12 - IVF #1 = BFN. 07/20/12 - WTF Appt -Told by our RE to quit IVF. Second Opinion from RE is good. IVF#2 - November 2012. Estradiol Pills Started 11/6. Stims start 11/16. ER 11/26 - 7 eggs retrieved - all mature. 4 fertilized with ICSI. ET 11/29 Transferred 3 embryos. Beta is 12/10. 1st Beta 81 2nd Beta 160 and 3rd beta 360!!! First U/S 12/21/12 - We saw one beautiful gest. sac. 2nd U/S is 01/04/2013 - H/B 183 02/05/13 - NT Scan - everything looks good and IT'S A BOY!!! EDD 08/19/12

I had 4 embies and we pushed them to a 5dt. All 4 made it, but only 1 to blast. We transferred the 2 best ones. I would probably be very worried about transferring 3, but it's a tough call. I would be fine with twins, but at our age, carrying triplets would be very difficult on us and the babies. I'm not sure what I would do either, but good luck. 


Me: 35, DH: 36, ttc: 3+ years, dx: unknown
10/11: after 2 years, saw a RE, FSH 5.4
11/11: BFP! (surprise after thyroid med & normal hsg),
12/11: missed m/c after 7 week u/s,
1/12: D&C,
6/12 IUI #1: clomid, 5 mature follicles, excellent sperm,= BFN,
7/12: IUI #2: clomid, 3 mature follicles, excellent sperm= BFN,
8/12: IUi #3: clomid, 5 follicles, = BFP!, C/P
10/12 IVF #1: FSH 5.4, estradiol AFC: 14, long Lupron 150/bravelle75,5R/5M/4F, all 4 made it to 5dt, 1 blast and 1 8 cell transferred. No frosties. BFN
IVF #2: AFC 21, MD lupron 12/25, 225bravelle/150men, 4R/4M/3F, 5dt of 1 blast BFN
IVF #3: natural start antagon, starting 3/30, ER 4/11, 12R,11M,10F

The difference is that Mayzie is 2 yrs old and her DH has severe MFI-- totally different than unexplained.  I'd put all 3 back (see my response on >35 for more info on why)

I asked my RE about 5dt at my WTF for my failed IVF.  He said that the main reason that they do a 5dt is just to pick out the best embryos if you have multiples to choose from.  In my case, we are pretty much putting in any and all that make it to maturity so it is not important to wait for a 5dt.  He also said that sometimes embryos won't make it to 5dt that may have made it if they'd been transferred because the uterus is a much nicer place for embryos to grow than a petri dish.  We had chosen to not transfer a 5 cell B embryo last time (because we had 3 beautiful 7 and 8 cell As) and it did not survive to freeze.  He said there's no guarantee it would've done well in utero but he thinks it might have had a better chance.

ETA:  I would also put in all 3.  We will put in up to 5 this time if we have them (the maximum allowed for my age group).

In your position, I'd likely put back two, but it's a super-tough call to make.

You had one failed cycle, which is not the end of the world, but you also seem to have some issues with fertilization, which could hint at egg quality. But I am always concerned about multiples, mostly because of the higher chance of miscarriage. I'm more afraid of miscarrying again than I am of having a failed cycle. Whatever you choose, I'm rooting for you. GL!



TTC #1 for about 3.5 years
ME: 37, PCO, immune issues?, teflon uterus (my added diagnosis)
HIM: 38, Perfect. (Showoff.)
Month before starting Clomid, spontaneous pregnancy. m/c at 7 weeks and D&C.
Four months of Clomid, one IUI, with Intralipid each cycle: BFNs all
IVF 1.0 July 2012 -- Lupron, bravelle, menopur, estrogen, progesterone supps, intralipid. 14 ER, 14 fertilized, 1 blast transferred, 2 on ice. BFN
FET 1.1 -- 10/2, transferred our 2 blasts. Lupron, PIO, estrogen, baby aspirin, prednisone. BFN.
IVF 2.0 December 2012 -- Bravelle, Menopur, Ganirelix, Prednisone, Metformin, estrogen, progesterone supps. 22 eggs retrieved on 12/14, 17 mature, 15 fertilized. OHSS anticipated so ET cancelled. 3 embryos frozen.
Broke up with old RE. Moving on to new dude. Maybe the change of scenery will be just the trick for our frosties. On BCPs for possible April FET.

I agree with all of the ladies. We had 3 originally but ended up with 2 that made it to transfer. If all 3 had continued to divide, we would have transferred all 3.

TTC since June 2011
DX: DH (30) severe MFI, severely low count & low motility
Me (32): all clear
Appt with Urologist 5/21/12: exam, ultrasound, bloodwork all normal.
Testicular Biopsy with TESE on 6/8/12. good sperm found! (left side only) froze sperm, failed thaw test :(
Orientation for IVF/ICSI on 6/13/12. Waiting for the green light following biopsy results...results show adequate sperm production both sides.
2nd SA 6/18/12: sample is "adequate for ICSI"
Plan: IVF/ICSI July 2012!
ER: 7/26/12. 15 eggs retrieved, all mature.
TESE/TESA/aspiration from epididymis, no motile sperm found :(
froze all eggs, the saga of praying for good sperm continues.
8/3/12: 2nd opinion from MFI uro on biopsy slides. Suspects "partial late maturation arrest."
Plan: more SAs, third biopsy/TESE with frozen back-up either from DH or DS.
SA 8/17/12: Zero sperm
SA 8/23/12: Zero sperm
9/26/12: SPERM FOUND! 15 eggs thawed, 12 survived and were ICSI'd, only 3 fertilized normally. Refrozen as embies and will thaw in Nov. Please survive and grow!
All 3 survived the thaw on 11/15/12!
FET 11/17/12: transferred 2, one 4B, one 4C. Beta 11/30:BFFN
moving on to DS
DIUI#1 2/18/13,50mg Clomid cd 3-7, Ovidrel trigger, Beta 3/4/13: BFN.
DIUI#2 3/19/13, 50 mg Clomid cd 3-7, Ovidrel trigger, Beta #1 (14dpiui) 4/2/13: BFP!!!! 150. Beta #2 4/4/13: 420 Beta #3 4/8/13: 2691. Beta #4 4/15/13: 15,086
1st u/s 4/8/13 shows one gestational sac
2nd u/s 4/15/13 shows yolk sac, fetal pole and early heartbeat
3rd u/s 4/25/13: measuring right on track. Heart rate 148 bpm

I would transfer all 3 embryos if your ok with multiples and all of the risks that can come with a multiple pregnancy.

FX that is works for you and you get your take home baby(ies).

Me (33)- unexplained MH (35)- MFI due to Vericocele. Straight to IVF w/ICSI 9/12, Antagonist = BFN c/p, poor quality blasts, tested sperm dna fragmentation. Results all good, yay! New RE for IVF #2- 1/13., Long Lupron. ER on 1/22 -10R, 9M, 9F. Transfer on hold due to overstimming. FET in Feb. 2 frozen blasts. Another BFN & another c/p. RPL testing all negative besides MTHFR gene, vericocle repair surgery 4/12/13-Bilateral Grade 3 Vericocele found & fixed, IVF #3 with PGS 4/13- 11R, 9M, 9F. Transferred 2 normal=BFN, 3 normal on ice Everyone Welcome!!!

I would be inclined to say put two back, and maybe see if the third could make it to blast? If it does you can maybe freeze it. But again, totally a personal choice. Best of luck!

I'm sure this is a very difficult decision for you. As a women with a less than straightforward infertility history, I can sympathize. I would have to be as aggressive as my RE would let me. Wish you the best.

When we were doing IUI we'd decided that if we ended up with 3 or more we'd more than likely do selective reduction, so I personally wouldn't want 3 transferred with IVF - maybe two and try to freeze the third?  But it's really up to you and whether you're okay with the possibility of triplets, and the risks that come with them!

I dont really have a opinion since DH and I are absolutley against multiples if we can help it, therefore we only put back one at a time. I believe that my age is a little different than yours though. I wish you the best with your decision and good luck with your embies!
