Low AMH & IVF w/ PGD/PGS

Those of you with a low AMH, did your RE suggest IVF with PGD or PGS the first time around?  My AMH is .23 and I'm 27.  We are just in the beginning stages of learning about everything IVF and are looking to start early 2013 but wanted some advice from you all as well.  Looks like the PGD/PGS process will take an extra 6-8 weeks.

TTC since Nov 2010 | Me: 28, DH: 29 | SA & HSG Normal
IF Treatment started December 2011
Late DX DOR after 10 months of treatment | AMH-.23, FSH-11.1
Stag II Endo removed in July 2012
5 Rounds of Clomid(50-150mg), TI & IUI#1 = BFNs
3 Rounds of Femara(5mg) & Prog, TI, IUI#2, IUI#3 = BFNs
Supplements include DHEA, CoQL, L-Caratine, Vitamin C/D/B3, Fish Oil, and Prenatal
March IVF cycle canceled due to cyst
April IVF #1 - Lupron, Saizen, Menapur, Gonal-F, baby aspirin and dexamethasone | AFC 8 >> 9R, 4M, 2F w/ ICSI...looking to do PGS testing...Stopped dividing at day 3.
Retested AMH, dropped to undetectable at <.16.
Trying to decide what to do next...

PGD should not add any time to your cycle.  I don't know anything about AMH, but we did IVF with PGD - eggs were retrieved and fertilized.  On day 3, viable embies were biopsied and the cells were sent to the PGD lab.  On day 4, they were tested and on day 5, results were conveyed to my RE and we went in for ET.

You need to be in contact with the lab (or your RE does) about a month in advance, but you can do that now (or soon).  GL!

We are doing PGD, but it is for a genetic issue. My RE does consider my AMH borderline low though, at 1.17. There are different types of PGD and we are doing aCGH, which doesn't add any time on the cycle. Other types of PGD like FISH can take longer because a specific probe has to be developed. If you aren't doing PGD to test for a specific chromosomal or genetic issue, aCGH is best because it tests all chromosomes.

I know that doesn't answer your question at all, but thought it might be helpful information!

It's probably because I'm in a relationship with a partner who does have great eggs, but my RE doesn't even recommend we try IVF with my eggs because my AMH and AFC are so low.  Perhaps the RE was trying to save us $ because there really aren't likely to be enough follicles to even stimulate for me...  but it wasn't even one of the options on the table for us.

That said, she did suggest using my partner as an egg donor for me, and doing PGD as our first line IVF because my partner has a strong family hx of invasive breast cancer.... But if you don't have a known genetic condition, I'm not sure I would spend the extra $, time and effort... 

 

 


Same sex couple - 31 years old - donor sperm, carrying our first due to dx of DOR/POF in Nov. 2012.

Low AMH, AFC - 6, Normal FSH, SS-A (RO) Antibodies (Autoimmune issues), tubes clear

IUI# 1 - December 19 - Done too early (24-36hrs before Ovulation) - BFN

IUI# 2 - January 16 - Femara + HCG trigger - BFN

IUI# 3 - March - 150-225iui Puregon, stimmed 16 days with only 1 follicle - nearly missed surge so no trigger - Progesterone - BFN.

IUI # 4 - "Pokey-Pokey" April 9 - 100mg Clomid CD 3-7, HCG Trigger + Progesterone - 2 follicles! IUI done after one O'd and before the 2nd one did. - BFN

...Moving on to trying to get my Partner pregnant with donor sperm. Hoping to come back and try with me, or do IVF with my DP's eggs eventually. My nerves are too frazzled for now

We talked about it, b/c I've had a history of miscarriages.

But, with my low AMH I only produced 5 embryos, and only 4 made it to day 3-so we just didn't have a enough to go foward with PGD to have it make sense.  I talked to two different RE's and both felt we needed at least 7 or 8 embryo's to consider PGD.

My AMH is 0.6, and my RE has not mentioned PGD to us.  We're in our first IVF cycle now.  Good luck!

TTC #1 7/10, Me (31)- Dx DOR (AMH 0.6); DH (30)- normal

I am around your age, 29 with DOR. My doc said that it wasn't necessary for us to go that route. Unless there are known issues that you need to look out for it isn't necessary. We may not have many eggs, but based on our age they should be good quality!

Thanks ladies.  Not 100% sure if we will move on with PGD, but looks probable. RE said it's really the only way to ensure better quality transfers since they can only see so much under a microscrope.
