Update: Our phone consult with C.CRM

My Update: We had our phone consult yesterday late in the afternoon. It went really well. Our doc, Jennifer Brown, was very on top of things and had reviewed our charts and history. She asked us to talk about how we got to the point we're at and so I gave her our history, in my own words.

She was familiar with things I called out and noted that "she had seen that" several times, which was awesome and made me feel like the trouble of getting all our records was totally worth it.

She thought that we would be very good candidates to do IVF with PICSI with CCS testing, to see if we can find any normal embryos and then move to a FET with 1 or 2 of those embryos (if there are any.)

She wants to really stim me up good, so that we're getting most of the 20 some eggs we've usually see to a mature stage – she'll do this through a tweak of the antagonist protocol that CCRM uses. She'd also like me to stim longer than my usual 9 days to push it so that this can happen. Since we're doing antagonist and they won't be doing a transfer right away, I'm at less of a risk for OHSS.

She thought that I was probably still "unexplained" as my "light PCOS" doesn't really line up with much medically besides a higher AFC. She thought that my "DOR" also wasn't adequate because of my higher AFC and good response to stims.

As for DH's high fragmentation rate, we'll use PICSI to ferret out the best swimmers and hopefully we'll have a high fertilization rate.

Having repeated losses means having abnormal embryos for some reason(s), so the CCS testing will help us figure out if it's possible for us to have normal embryos. Also they'll run another RPL panel on me.

They'll check for blood flow and do a hysteroscopy (another kind of 'scopy to add to my bucket list!) and I'll have a endometrial biopsy to check for the beta 3 integrin protein.

I'll have to have my first mammogram because I am old (over 35). YAY boob squashing!!! But I can do that here, with my normal doc.

CCRM wants a lot of bloodwork re-run in their lab and all the uterine under the hood kinds of things, which is cool. Much of the testing we've done will "count" and we don't need to redo things again. Thank Jebus.

I asked her straight up if this was a reasonable thing for us to do and she said yes, no doubt.
So…
We have our One Day Work Up (ODWU) scheduled for the 4th of January! From there, depending on what happens with our tests, I'd guess we'd be looking at a mid January cycle start date or more likely, mid-February.

It feels so good to have a plan!

Wooty Woot from me too! I'm so glad that you have a good plan. It really sounds like CCRM has taken a good look at your case and has something really good in mind for you. I really want this to be it for you. I'm cheering you on big time!!!

Yes! Our monitoring will be done here and my doc/RE is totally on board. We'll need to go out to Denver toward the end of stims for monitoring when we're getting close and then for the retrieval. Then, if we have a normal embryo, we'll go back for the transfer at a later date.

Oh, good! I'm glad your RE is on board and you don't have to go to a clinic that's never treated you before.

That sounds great. Well, not great in the sense that you're looking at a few billion trillion tests and such from here on out to January, but great in the sense that it really seems they've got a good handle on your situation and are developing a plan for tackling it.

I've always been curious why it is that places like CC.RM have such good success rates and other clinics don't. Maybe it's how thorough they are.

TTC #1 for about 3.5 years
ME: 37, PCO, immune issues?, teflon uterus (my added diagnosis)
HIM: 38, Perfect. (Showoff.)
Month before starting Clomid, spontaneous pregnancy. m/c at 7 weeks and D&C.
Four months of Clomid, one IUI, with Intralipid each cycle: BFNs all
IVF 1.0 July 2012 -- Lupron, bravelle, menopur, estrogen, progesterone supps, intralipid. 14 ER, 14 fertilized, 1 blast transferred, 2 on ice. BFN
FET 1.1 -- 10/2, transferred our 2 blasts. Lupron, PIO, estrogen, baby aspirin, prednisone. BFN.
IVF 2.0 December 2012 -- Bravelle, Menopur, Ganirelix, Prednisone, Metformin, estrogen, progesterone supps. 22 eggs retrieved on 12/14, 17 mature, 15 fertilized. OHSS anticipated so ET cancelled. 3 embryos frozen.
Broke up with old RE. Moving on to new dude. Maybe the change of scenery will be just the trick for our frosties. On BCPs for possible April FET.

Hysteroscopy 3/2011 2 Polyps removed 4/2011
DH on Clomid / Arimidex to address hormonal issues - Sperm found July 2011!
IVF #1 September 2011 - ER 9/8/2011- 8R, 5M - icsi - 1 made it to transfer - BFFN :-(
IVF #2 Lucky November 2011 - ER 11/25/2011 - 15 eggs!, 13 mature, 10 fertilized with isci - 7 made it to transfer! Transferred 1 - 8 cell and 2 - 7 cell. BFP!!!!
1st U/S - Twins! 2nd U/S - 3 heartbeats!!! - HOLY COW!!!
Prayers for our take home babies!
2 babies born on 3/27/12 and lost. Last baby born on 3/29/12 and lost at 20 weeks.
My angels grew wings shortly after birth.

IVF #4 October 2012 - New RE. Doing PGD Testing. 8 R, 7F, 2 made it to 5 day blast for PDG testing. Both Normal (Thank you G-d!!!) 1 boy and 1 girl
transferred one beautiful 5day blast (girl)
Please G-d the almighty one - let this be our take home baby!

Just wants to say I'm glad your CC.RM consult went well.  I have Dr. Su.rrey, but Dr. Br.own did my both my ER and ET, I liked her a lot.  Good luck on your ODWU and with your cycle! 


TTC #1 7/10, Me (31)- Dx DOR (AMH 0.6); DH (30)- normal
BFP on Cycle #8. Natural M/C at ~8 weeks
IUI #1 and #2 (8/12, 9/12)- 50 mg Clomid + Ovidrel = BFNs
IVF #1 (EPP) 13R/10M/9F, 5DT 1-4AA blast, beta #1-148, beta #2-322, 5 frosties!
3/18/13 - It's a Girl!
4/18/13 - A/S- baby looks perfect, but partial placenta previa

04/07/11- PCOS Diagnosis w/GYN. Put on Metformin (1500 mgs) 04/25/11- 1st consultation with RE (Confirmed PCOS & Anovulation)
05/09/11-HSG-All clear! 05/13/11-SA-Normal. Found out on 2/14/12, low Morph (1%).
06/17/11-10 days of Clomid (100, 150) No response.
07/10/11-03/20/12- 5 Injectable (Follistim) IUI cycles; 4 BFN, 1 C/P. (stims ranged from 11 days to 25 days)
05/28/12-Diagnosed with Hypothyroidism. 75 mcg Synthroid.
IVF with ICSI in June. BCP June 6-15. Stims (Follistim + Menopur + Ganirelix + PIO) 6/19-6/29. Trigger 6/30. ER 7/2: 47 retrieved, 45 mature, 42 fertilized with ICSI! 18 frosties. ET 7/8 (1 6AA) BFP! (+ HPT 7/13, 5dp6dt). 7/16 Beta #1: 123. 7/18 Beta #2: 252. 7/25 Beta #3: 3488; Ultrasound confirmed gestational & yolk sac!. 7/27 Beta #4: 8247. 8/2; Heard heart beat!! 6w3d=122.Beta # 5: 30,890. 8/9 U/S shows baby measuring right on track with a hb of 158. 8/16 Graduated from RE! HB 181. Elective ultrasound on 10/10 to find out gender. It's a boy!! A/S on 11/13 showed baby boy perfectly healthy & weighing in at 1 pound. 11/29 Diagnosed with GD @ 23 weeks. Collin was born on March 27, 2013!
