Unexplained IF with IVF failure testing?

Hey all, I'll prob XP this onto PAIF for their input too, but I'm curious about everyones experiences here.  I know I'm slightly putting the cart before the horse since my beta is tomorrow, but so far all the tests have been BFNs. For those of you that are unexplained and have had a failed IVF cycle, did your RE do any additional testing?  I have had the cd3 labs, afc u/s, hsg and an amh level (I'll find out what it is tomorrow) and multiple SAs & hormone levels on DH.  Is there anything else I should consider?

Feb'12- Clomid/TI with CD3 &21 labs(normal)& SA-count 6mil/ml,morph/motil OK-BFN
April'12 Ultrasounds,HSG -clear, slight acurate uterus
5/14 IUI #1-50 mg clomid-post wash total motile count 19.3 prometrium 2dpiui= BFN
6/11-IUI #2-100 mg clomid & prometrium post wash count total motile 17.5 = BFN
7/6 IUI#3 with 100 mg clomid, estridiol, prometrium. post wash total motile count 23.3 = BFN
8/30IVF#1 BCPs, Follistim, Menopur,Ganerelix,Novarel.
ER 9/11-8R, 7M, 5F.
ET 9/14 2 embies transferred. 1 10cell Grade 4, 1 8cell Grade 4. No frosties. BFN
IVF#2 BCPs, Follistim, Menopur, Ganerelix, Novarel, Baby Aspirin
ER 12/5-16R, 12M, 8F!
ET 12/10 5dt! 1 fully expanded blast & 1 early blast. No frosties. BFN
2/13- Saline sono revealed a polyp. All additional labwork coming back normal. Genetic screen revealed DH has MTHFR deficiency & I am a carrier for it.
3/13 hysteroscopy & polypectomy
3/27 Began working with Reproductive Immunologist Dr. Kwak-Kim. Found High NK cells, High DHEAs, slight hypothyroidism, tendency toward blood clots (High PAI-1)Began synthroid & metformin 4/19.

My RE hasn't done additional testing, all she said was sometimes these things happen (extremely frustrating) So I decided to call my embryologist and talk to then about my quality of embryos in more detail. Which I've come to my own conclusion that my TE rating wasn't that good - it's the cells that have to do with implantation and placenta forming. That's the only thing that I can think of but who knows.Confused

TTC since 9/10 consult 1/12 All testing came back fine Diagnosed with Unexplained infertility All treatments OOP 3 medicated IUI's All BFN IVF - ER 11/14 24R, 15M, 13F ET 11/19, BETA 11/28 BFFN --Next step FET in Jan ET scheduled for 1/24. We were unexplained and had failed IUI's etc. When they went to begin testing for the IVF cycle they discovered a polyp not very big in my ute. They said it could or couldn't be the cause of my issues. Either way I had to have it removed by surgery before beginning IVF. It was a minor out patient procedure, similar to ER. We will never know for sure if that was the cause or not, they seem to think now it was, since both embryos we transferred implanted and stuck. We had tried for 2+ years before with meds, injections, IUI's, TI everything and no luck. I think it was a saline hysterogram or something that they discovered it through. Just a thought for you to ask about if you haven't already done so. I would have never thought that before either. GL and I hope your pee sticks are wrong!

Thank you.  I haven't had that test yet, but I'll add it to my list to ask him about.  Were they able to see the polyp on a regular u/s?

After my failed IVF cycle, they didn't do any additional testing but for this cycle we added in an endometrial biopsy to see if that would help with implantation.  FX that it does because it hurt like a b*tch!!

Keeping everything crossed for you that you get a positive beta though!!

Thank you.  I haven't had that test yet, but I'll add it to my list to ask him about.  Were they able to see the polyp on a regular u/s?

Apparently no. It was part of the testing to do the shared risk option of IVF, I am pretty sure they do it for any IVF cycle though. I know several people that have had it done, some have nothing some have a polyp or two. I am almost curious to see if that was the problem and do TI but we had wasted two much time and we were getting too close to the holidays to risk.

I'm glad you asked this!  We are not unexplained (MFI), but there were no sperm issues that we know of with the IVF and all of my mature eggs fertilized normally.  Before we do a FET, I want to be prepared to consider any other testing that we may need to do, especially on me, since 2 almost perfect embroys didn't take.  I would second PP's recommendation for a SHG.  This was standard prior to doing our IVF to check for polyps and other uterus issues that may not show up on an ultrasound.  I'm interested to see if anyone else has any other suggestions!  At what point is a lap usually recommended to check for endo?

I had wondered about a lap as well.  Not to be all Munchhausens about it, not like I WANT to have all these painful tests done, but I don't want to go through another cycle if theres something I could be doing to help first.

We were unexplained but after having 23 mature eggs and only 1 fertilize naturally we realize that we need to ICSI right from the start instead of only using it as a rescue technique. Plus there was a tweak in the medications too. I think for us if 2 fails we will question additional testing.
I hope you can get some answers.

We were unexplained but after having 23 mature eggs and only 1 fertilize naturally we realize that we need to ICSI right from the start instead of only using it as a rescue technique. Plus there was a tweak in the medications too. I think for us if 2 fails we will question additional testing. I hope you can get some answers.

Thank you.  With IVF#1, we had to ICSI them all.  With #2, we had enough to do an ICSI split (I was so happy about it!) but still nothing to freeze.

Feb'12- Clomid/TI with CD3 &21 labs(normal)& SA-count 6mil/ml,morph/motil OK-BFN
April'12 Ultrasounds,HSG -clear, slight acurate uterus
5/14 IUI #1-50 mg clomid-post wash total motile count 19.3 prometrium 2dpiui= BFN
6/11-IUI #2-100 mg clomid & prometrium post wash count total motile 17.5 = BFN
7/6 IUI#3 with 100 mg clomid, estridiol, prometrium. post wash total motile count 23.3 = BFN
8/30IVF#1 BCPs, Follistim, Menopur,Ganerelix,Novarel.
ER 9/11-8R, 7M, 5F.
ET 9/14 2 embies transferred. 1 10cell Grade 4, 1 8cell Grade 4. No frosties. BFN
IVF#2 BCPs, Follistim, Menopur, Ganerelix, Novarel, Baby Aspirin
ER 12/5-16R, 12M, 8F!
ET 12/10 5dt! 1 fully expanded blast & 1 early blast. No frosties. BFN
2/13- Saline sono revealed a polyp. All additional labwork coming back normal. Genetic screen revealed DH has MTHFR deficiency & I am a carrier for it.
3/13 hysteroscopy & polypectomy
3/27 Began working with Reproductive Immunologist Dr. Kwak-Kim. Found High NK cells, High DHEAs, slight hypothyroidism, tendency toward blood clots (High PAI-1)Began synthroid & metformin 4/19.

I had a rpl test panel. I know you thankfully haven't had losses, but sometimes that can pinpoint problems. We've had karyotyping done to check for any DNA issues. We are considering PGD testing with ivf#2 to rule out any DNA issues with the embryos.


Me: 35, DH: 36, ttc: 3+ years, dx: unknown
10/11: after 2 years, saw a RE, FSH 5.4
11/11: BFP! (surprise after thyroid med & normal hsg),
12/11: missed m/c after 7 week u/s,
1/12: D&C,
6/12 IUI #1: clomid, 5 mature follicles, excellent sperm,= BFN,
7/12: IUI #2: clomid, 3 mature follicles, excellent sperm= BFN,
8/12: IUi #3: clomid, 5 follicles, = BFP!, C/P
10/12 IVF #1: FSH 5.4, estradiol AFC: 14, long Lupron 150/bravelle75,5R/5M/4F, all 4 made it to 5dt, 1 blast and 1 8 cell transferred. No frosties. BFN
IVF #2: AFC 21, MD lupron 12/25, 225bravelle/150men, 4R/4M/3F, 5dt of 1 blast BFN
IVF #3: natural start antagon, starting 3/30, ER 4/11, 12R,11M,10F

After my failed IVF cycle, they didn't do any additional testing but for this cycle we added in an endometrial biopsy to see if that would help with implantation.  FX that it does because it hurt like a b*tch!!

Keeping everything crossed for you that you get a positive beta though!!

February 2013--Found a new RE but first back to the Endocrinologist for more diagnostic testing
Absolutely no idea when we can cycle again...looking into DE as well if IVF#4 fails again
PAIF/SAIF encouraged...give me some hope!!
