Femara and ovulation... confused...

I posted a few days ago about whether or not I O'ed, but my temps never went up.  Took Femara for the first time this cycle.  No, my doc did not do a CD12 u/s.  My dr. office doesn't do an u/s until you are on your second cycle (or 3rd or 4th, etc).  Don't know why, but they just don't.  Anywho, I haven't O'ed yet, or so they say.  I went in yesterday (cd17) because they were concerned I wasn't O'ing, and just wanted to check things out.  I usually O on my own no later than CD15.  They put me on Femara because they were just trying to improve the quality of the egg since I've lost two LO's now.  When I went in yesterday, they said I had three measurable eggs, one was about 16mm, one was 10mm, and I can't remember the other but it was smaller.  They said the one that was 16mm would probably be the only one to get any action, and that it would take 3-4 more days for it to be released.  But then this morning, I got a temp spike.  I'm pretty sure I couldn't have O'ed yesterday because 16mm is too small for it to have released, right?  Is it at all possible I had a 4th egg that already dropped before I went in there?  Sorry if that is a dumb question, but this part of TTC is still new to me.  Also, the medical assistant said it is common for Femara to push ovulation back.  Has that happened to anyone else?  I would think if anything it would make you ovulate sooner.  Somebody help, please!  TIA!

Femara pushed mine back and caused a cancelled IUI cycle, and the only time I have had a cyst was after that cycle. I was O'ing on my own CD14-15 prior to starting meds.

16 is too small and it would be an immature egg. If you had already ovulated they would have seen that. Well, maybe. If they know what they are doing they would see that, but that is questionable.

Ultrasounds override temps. No one should ever need to temp on a medicated cycle because everything should be determined via u/s. When are you going back in for a follow up? Are they checking your progesterone levels at 7 dpo?

Are you seeing an OBGYN or an RE? Your doctor should be doing monitoring ultrasounds even if it's your first cycle on meds.

Ultrasounds are more accurate than temps, especially on medicated cycles because the meds can mess with your temps. I would request another ultrasound in a few days to see how the 16mm follie is progressing. They should be able to tell if you have already ovulated. I have ovulated around the same time as normal on medicated cycles but some people find that it changes.

That being said, I did have 1 cycle on Femara where ultrasounds never showed any follies bigger than 8mm. The cycle was eventually cancelled due to no response but I did have a temp jump around the time I normally ovulate. (I started temping halfway through the cycle when I found out I wasn't responding). My doctor did not think I ovulated that cycle and offered me Provera but I wanted to see if I got my period on my own. I did, right when right when I expected it based on my normal LP. I had ultrasounds both before and after the temp spike that didn't show anything. I have no way of knowing for sure, but my guess is that I released a small, immature egg.

TTC #1 - Stopped using BC March 2011, officially TTC since August 2011
Dx: PCOS & mild hypothyroidism (me) & MFI
2 cycles Femara + trigger = BFN/no response. 2 cycles Clomid + trigger = BFN
IUI # 1 = BFN
Repeat SA - low motility & morphology w/ 99% bound by antisperm antibodies
Recommendation: IVF w/ICSI

No follow-up was scheduled.  They just told me I should probably ovulate in the next 3-4 days... And that was it!

I had ultrasounds both before and after the temp spike that didn't show anything. I have no way of knowing for sure, but my guess is that I released a small, immature egg.  

It's an Obgyn.  I haven't been referred to an RE yet because I've only lost two LO's.  My dr. says they don't refer until you've lost 3.  He offered me Femara after the first loss, but I said no.  After I lost the second one it was much more traumatic), I thought I would try to improve my chances and go ahead and try the meds.  For some reason they don't monitor the first cycle at my doctors office.  They say if it doesn't work the first time, they monitor any cycles after the first one.  I would like to ask for another one, but as it turned out, the one yesterday was not covered by insurance and I had to pay out-of-pocket for it.  So I can't afford to do another one right now, unfortunately.

GM is wise and unfortunately if you can't afford the monitoring, then you should cease all of the meds for now.

My first Femara cycle was cancelled due to poor response, my second Femara/Follistim was cancelled because of the same.

I went in this cycle on CD 3 for baseline u/s and found 2 huge cysts. Had I not been monitored, bad things could have happened. I am in pain now because of it.

These meds require monitoring, there is no way around that.

Had I known it wouldn't be covered my insurance, I wouldn't have done it, trust me.  Also, I didn't realize all of the monitoring that was supposed to happen until a few ladies on here told me.  The doctor seems to not think it needs monitoring the first cycle for whatever reason.  I will definitely talk to them about cost before doing another round of Femara.  Thanks for the info, and sorry about your cysts... That sucks!

Has your doctor even done any testing to see what the issue is, or if there even is one? I'm sorry you have had losses but 2 is not necessarily indicative of a fertility issue.
