Ladies with a failed IVF, come in!!!

I'm in the process of scheduling my WTF appt. I'm just not sure where to go from here. I don't know what I should be asking or what I should be looking for. Can they really tell me anything beyond "It didn't work"? They said everything on paper looked great, so how can they really give me a why? 

I know that I would like to do another IVF cycle, but I'm not sure if I want to stay at my current clinic (NH) or go to one of the big clinics in NYC like Columbia/Cornell/NYU. I was originally a patient at Columbia but moved over to NH to do the clinical trial. Now that the trial isn't a factor, do I stay, go back to my other RE, or go somewhere totally new. Being OOP, I want to make sure I'm looking at the whole picture and not just going with the easiest and most convenient. I don't feel like doing another Mini cycle is really beneficial for me due to my response with the last one and Natural/Mini is what NH specializes in. And if I do conventional, why stick with NH, when there are other clinics with better success rates out there?

What did you ask at your WTF appt and how did you decide to stay or go? Any insight would be greatly appreciated!!!

Sorry about your failed cycle. It can be very tough. I let my RE do the talking at my wtf appt mostly. I just asked what she would do differently next time and if she saw any reason why it failed. As for changing clinics, I would say just to go with your gut on that. If you feel like they are doing everything they can for you and if you trust them and their judgement, then stay. If you don't feel like they have a solid plan, then I'd go... Good luck to you.

SInce we knew that we had severe MFI, we knew that there would be many challenges, and most likely if embies didn't work, prob. a chromosonal thing due to sperm issues. The 1st cycle after a big ol BFN, when they called me I wanted to try again, they suggested in my case because they felt I would respond better since they knew at that point how my body would respond. I had never taken any fertility meds prior to that, so although the 1st round went well, it was a practice run realistically.She was absolutely correct, I suppressed better, and had more eggs. So we went right into the 2nd, After the early M/C at 5w5d,

 I had a WTF appt, and just confirmed my thoughts, which was sperm issues, and that they typically will stop growing before or after transfer until you find the right one. She advised that my eggs were very good quality, I had enough, nothing else I could really do or change on my end other than maybe 3dt or freezing some if we could. Just had to keep trying, and she was hopeful because we made as far as we did. I had to trust her, because she had been absolutely dead on with everything she suggested. Even when I had friends doubt doing it like that.

I will say, that she did make me feel better with staying with her because, she randomly called me day after christmas to let me know she ripped my chart apart again, and saw that there was signifacant improvement, and what we might have done so that we could keep doing it. Only change was complete bed rest and his supplements along with his clomid prob kicked in (3 months).

I say all that to tell you I felt like she really had my back, and the reason I decided to stay with my re which is smaller is several factors.

1.) I liked that she kept encouraging me even at the WTF appointment, and even followed up several times after.

2.) researched showed that the fact that we made it to 5dt 2x with his severe issues, was really good for her group. Some of the bigger groups aren't always better, and that they sometimes will push for 3dt. It takes a lot of patience, good staff, and good equiptment. Which clearly my dr. had, if our embies aren't going to make it or issues, nothing ANY dr. can fix, I feel comfortable with her and confident she had my best interest at heart so that is who I am sticking with. I have a very good repoire with the staff, they know my body.

Best of luck in your decision, so tough, I did think about going elsewhere. Maybe you could get a 2nd opinion. See how you feel after the appt. Like J&D said. I hope I made sense lol, I just had a large coffee, bahaahahhaha <3

Hi -- just wanted to say that I live in NYC and switched from Columbia to NYU for IVF #2, which I'm just starting. I thought Columbia was fine but figured if IVF #2 was my last (which is likely) I should go with my "dream" clinic, even if it cost a bit more. My research led me to NYU, and I've been really happy so far.  My thinking is that if it doesn't work at NYU, at least I feel like I've been to the best, or at least the clinic I think is the best!

I was just coming around to ask this same question.  My WTF isn't until the 21st.  I don't know how I'm going to wait that long.  I'll definitely be making a list of questions. 

One other question I had for you ladies: were there any additional tests that were recommended after a failed cycle?  Or insurance changed 1/1 and now only testing is covered, so I'd be willing to do more of that if it might help.  I want our FET of our 2 frosties to be the best that it can be. 

Right now we are just MFI (low morph and borderline motility).  Shouldn't the IVF with ICSI taken care of these two issues?  Was there something with me that cause the late/poor implantation?  I had quite a bit of bleeding (heavy spotting) from the day that I suspect that I implanted (late at 7dp3dt) and continued until the loss started and it got worse.  I'd like to know what caused that or if it was etopic.  Can they tell that now?

Sorry that was a random blurb of info and questions.  I just figured it would be better than starting another post on the same topic :)

If it was up to me with my personal situation I would probably switch doctors but we have different situations. I think you need to weigh the pros and cons on this one. What do you like or don't like about your current RE And do your research on the new place you would want to go! Good luck the decisions are tuff!

Schwind, to answer the MFI question, ICSI will help with the motility and morphology aspect of it. Anotherwords, it takes the work of the sperm having to get into the egg out of the equation, especially if they not straight swimmers, and shaped odd. It does not solve however the issue if the sperm are deformed, chromosonal issues etc. I think that is where PICSI and the other methods come in.

As far as testing you could do pgd on the frosties you have, but that would be a huge risk since only 2, since you had bleeding from the beginning they could see if there is an implantation issue. I believe you had the steroid injection to help, not sure about the aspirin route. Insurance or RE, sometimes don't do further testing if M/C is not a recurrent issue. When they were drawing betas, they can see if hormone levels were good, they would addressed it. My Re, mentioned that they are finding more and more studies showing woman who have not had good experience with Fresh cycles, may respond better to FET, because the uterus is in a more natural state. With fresh cycles they are pumping you full of hormones, and your lining is not at it's ideal state. I know my Dr. mentioned with me if this one doesn't work we need to try frozen method.They will tell you if they believe ectopic or biochemical. FX for this for this FET round <3

They said my #'s were textbook and that my eggs wee great, etc.  Our problem was DH's sperm.  I was ready for round 2, but RE suggested DH try Femara for 90 days. We did see improvements from it and are so hopeful for #2 starting tomorrow!!!  It's so devastating...don't lose hope. Our post appointment was terrible...my husband and I both buckled over the RE's desk bawling.... we will all get through this!

We are trying this : I had two failed IVF cycles and I knew with IVF3 I wanted a second opinion, time to research options, etc.

At our second opinion appt with a new RE we felt *educated*, more so than our previous clinic. The new RE has a slightly different lab and approach to growing the embryos.  Very long story short : For IVF3 we decided to go with the new RE, new protocol (MDLF) and CGH genetic testing and only do a frozen transfer. We had success! Our goal was 1 genetically normal high quality blastocyst (for my age, that's doing pretty good).  So now, we are moving forward with IVF4..same protocol, same RE..trying for one more normal.  Hopefully, we'll do a transfer in March of this year!

Another vote for NYU here -- I cycle there and really like it -- my doc, the nurses, everyone.   I'm getting a second opinion elsewhere but only because I have unique issues.  If I end up cycling elsewhere I will really.really.miss NYU.

My best advice is that you should go to the best place you can reasonably afford and manage the logistics of.  We are also OOP and I work full time, so going out of the area is a big issue, too.  

I ended up leaving my former RE.  There were a variety of things I thought were not managed well with cycle 1 and 2.  I ended up getting an immunology consult w/ Dr. Braverman, consulting 2 other local RE's and one well known RE out of our area.  I decided to go with a local doc who is very meticulous and has helped about 4 of my friends and colleagues get KU.   

 I would suggest that you consult at least a couple new docs before deciding what to do.   


TTC #1 12/2009 BFP #1 1/2010, M/C 6 weeks;
BFP #2 6/2010, DD lost to congenital heart disease, we are heartbroken.
TTC #2 4/2011, Diagnosed MTHFR homo, FVL hetero; Four natural cycles BFN;
8/2011 Hysteroscopy, small septum; Acupuncture; Clomid IUI BFN; Follistim IUI BFN;

Our first wtf we let the re run the show and he pretty much answered our questions as he went along. Im sure hell do the same with our 2nd wtf but i have a list of things i want to check into so im sure ill have more to say this time.

We chose to stay w current re after cycle 1 failed because we like the office plus its location is better in regards to work and the frequency of monitoring. We had a much better response with cycle 2 and will stay with them for cycle 3. I just love my office, what can i say. Good luck w your appt decisions

Feb'12- Clomid/TI with CD3 &21 labs(normal)& SA-count 6mil/ml,morph/motil OK-BFN
April'12 Ultrasounds,HSG -clear, slight acurate uterus
5/14 IUI #1-50 mg clomid-post wash total motile count 19.3 prometrium 2dpiui= BFN
6/11-IUI #2-100 mg clomid & prometrium post wash count total motile 17.5 = BFN
7/6 IUI#3 with 100 mg clomid, estridiol, prometrium. post wash total motile count 23.3 = BFN
8/30IVF#1 BCPs, Follistim, Menopur,Ganerelix,Novarel.
ER 9/11-8R, 7M, 5F.
ET 9/14 2 embies transferred. 1 10cell Grade 4, 1 8cell Grade 4. No frosties. BFN
IVF#2 BCPs, Follistim, Menopur, Ganerelix, Novarel, Baby Aspirin
ER 12/5-16R, 12M, 8F!
ET 12/10 5dt! 1 fully expanded blast & 1 early blast. No frosties. BFN
2/13- Saline sono revealed a polyp. All additional labwork coming back normal. Genetic screen revealed DH has MTHFR deficiency & I am a carrier for it.
3/13 hysteroscopy & polypectomy
3/27 Began working with Reproductive Immunologist Dr. Kwak-Kim. Found High NK cells, High DHEAs, slight hypothyroidism, tendency toward blood clots (High PAI-1)Began synthroid & metformin 4/19.
