ULTA Question

Does anyone know if ULTA's insurance covers IF in all states that they have shops and how many hours a week you have to work to be covered? I am fairly certain IF is covered since their corporate location is in IL (a mandated state) but I would be working in TX (a mandated state that doesn't require employers to offer IF coverage, only for Ins Cos to offer it to the employers).

 

I really don't want to work retail, but I don't have a whole lot of options at this point. Sad I applied online so we'll see what happens.

Me; 35 DH 32 TTC for over 2 years First mini-IVF Sept 2011... Only 1 egg! ... BFN Switching RE IVF#2 May 2012 9 eggs and only 2 sperm, WTH! BFN IVF#3 Chemical... FML. 2 frosties. SAIF/PAIF ALWAYS WELCOME!!!!!

lol. I just did it also. Try "ULTA" and "insurance coverage" in quotes so the search engine specifically picks up these things only?

It depends if the coverage they give you is national or local. If it's a national plan and the insurer is based in IL, then yes. But sometimes companies will provide local coverage for employees in different locations - so for example, colorado employees have one insurance plan and wyoming employees have a totally different one, and then it might not be covered. 

Your best bet is to call the number on your insurance card and ask them what's covered.

That's what I figured. I think it's a national plan...I found their benefits page on a different website than their consumer site. Thanks!

TTC #1 for about 3.5 years
ME: 37, PCO, immune issues?, teflon uterus (my added diagnosis)
HIM: 38, Perfect. (Showoff.)
Month before starting Clomid, spontaneous pregnancy. m/c at 7 weeks and D&C.
Four months of Clomid, one IUI, with Intralipid each cycle: BFNs all
IVF 1.0 July 2012 -- Lupron, bravelle, menopur, estrogen, progesterone supps, intralipid. 14 ER, 14 fertilized, 1 blast transferred, 2 on ice. BFN
FET 1.1 -- 10/2, transferred our 2 blasts. Lupron, PIO, estrogen, baby aspirin, prednisone. BFN.
IVF 2.0 December 2012 -- Bravelle, Menopur, Ganirelix, Prednisone, Metformin, estrogen, progesterone supps. 22 eggs retrieved on 12/14, 17 mature, 15 fertilized. OHSS anticipated so ET cancelled. 3 embryos frozen.
Broke up with old RE. Moving on to new dude. Maybe the change of scenery will be just the trick for our frosties. On BCPs for possible April FET.

If ULTA self insures they do not need to provide IF coverage, despite the Illinois state mandate.  My DH works for one of the largest employers in Illinois, and they excluded fertility coverage for many years on that basis.  There are many loopholes to the law, unfortunately.

DOR and AMA
2/12-5/12: 4 IUI cycles = all BFN;
7/12: DE IVF # 1 (with ICSI)- 20R, 16M, 14F, 5DT of 2 blasts (4AB & 3BB); 6 frosties = BFN;
Lupus anticoagulant initially high, then found to be normal on hematology consult;
Follow up testing in September all clear;
Started synthroid for "high normal" TSH;
FET # 1- late October 2012- BFP on FRER; beta # 1- 21(low), beta # 2- 48 (still low), beta # 3- 132, beta # 4- 1,293; beta # 5- 5,606; last beta- over 100,000. First u/s 11/21- heard heartbeat
12/12- Officially an OB patient!
Level 2 ultrasound shows vasa previa and referral to MFM
Mandatory c section for delivery
Beautiful baby girl born at 34 weeks

Ugh! Now I need to find out if they self insure. I don't want to waste time going for an interview (assuming they want one with me) if they don't cover IF. Like I said I really don't want to work retail and the only reason I would is for IF coverage. Thanks for the info!

Ok, so I went back an reread the benefits page I found earlier and it says their insurance is an indemnity plan and not major medical. So from what I can tell that means its a crap plan. On to my next great idea...
