GhostMonkey (and others) - Secondary Infertility? How to know?

Anyone can answer, but I'm calling out GhostMonkey cause she's really good at 1) being blunt and 2) knowing her stuff.  But anyone with advice is free to chime in.

So, general rule is no need for fertility testing until a year of TTC UNLESS there are known medical issues.  Like a super short LP as seen in an earlier post or multiple miscarriages or whatnot.

Here's my situation (summary):

-PCOS diagnosed at age 14. Bloodwork, ultrasounds, the whole bit

-No periods (except my first one) without taking Provera, which I did from age 14 to age 28.  Metformin never did anything for me.

-At age 28, I lost 100+ pounds and periods started on their own, cycles around 35 days or so.

-Been charting BBT since January 2010, so on my 20th cycle charting.

-Got pregnant August 2010 on first cycle, thanks to 1) luck and 2) 18 months of charting so I knew when FW was really likely to be.

-Had a kid April 2012.  Not breastfeeding.

-Tried to get pregnant again right away (blessed by OB who preferred me to wait a cycle or two, but was okay if it happened earlier).

-Period returned 8.5 weeks PP.  Nothing remarkable. 

-Since then, my cycles are 34 days, 80 days, 50 days and currently on CD36 without ovulation yet.

-Me: 33.5, DH: 36, desiring of several kids 

 

My questions:

1) Do you still wait the year to talk to a doc?  Do you wait longer cause I "just had a baby" (even though it doesn't feel like I 'just' had a baby)?  Do you wait less time than that because cycles are so irregular or because there's a history of PCOS (even though it seemed much better and I got pregnant before)?

2) Do you ask for simple bloodwork sooner? 

I tried Dr. Google and while I found several articles on Secondary Infertility (that it's super common), none told me when it starts (ie: You have to be 6 months past your last pregnancy), so I'm wondering what your advice would be in this situation.

I don't think it would hurt to call at this point. Your cycles are to the point of being longer and irregular again. I don't know that the history of PCOS would play into the decision or not, but since you have a history of it and irregular cycles do tend to go along with that, that may be a starting point.

Worst they are going to tell you is that they would like you to wait out the next couple months before checking things. I don't think you are jumping to gun given your history and current cycles though.

We didn't start trying for #2 until DD was about 4... I was charting an not ovulating so my IF testing started around the 6 month mark. (after charting for 6 months prior to TTC and then the 6 months while we were really trying and only ovulating twice) My longest cycle was around 117 days. My OBGYN said he generally waits the standard 12 months before testing/RE referral unless there is an underlying issue.

That being said... you do have a history, so touching base with your doctor can't hurt!

TTC#2 Since July 2011
Me: 29, had two blocked tubes - left was cleared during lap&dye, right was unable to be cleared. PCOS & Stage 2 Endo. DH: 31, SA = perfect.
CLOMID: 4 rounds, 50mg + TI = BFN's.
FEMARA: 1 round = no response.
12-24-2012 : Laparoscopy, Softball sized cyst/endo/scar tissue removed.
Cycle #14 - Feb 2013 : 50mg clomid. Ovidrel Trigger. IUI on 2/14/13 = thin lining, multiple cysts.
Cycle #15 - no meds, still have cysts, no follicles. Boo!
Forced break. Continuous BCP for 6 weeks to give my jacked up ovaries a break.
Cycle #16 actively trying (May/June): Femara, TI = BFN
Cycle #17 Med break = ?

Thanks for the advice.  I do have an appointment for February, but was debating on keeping it or not.  I guess an easy thing is to take bloodwork.  If she wants a CD3 and a 7dpo, that could take a couple of months (easily) with cycles as long as I'm having.  I just didn't want to keep trying without seeing her if it wasn't likely that anything was going to change.  If I was having cycles with some regularity and still not getting a BFP, that's one thing.  But this goofiness is just hard to predict anything with, but if I had to burn the clock, then so be it.
