IUI IVF Questions advice to make it work for low motility?

I have low motility but have improved my sperm count by Clomid. Within the next 2 weeks we will be going for IUI per Dr's recommendations. My wife is questioning it that it will not work based on her online research. I told her it is step one. I take a multivitamin and am trying to see what else I can do to improve the chances to make this work. Is there a certain food I should start to take? etc. My wife checked out fine. My Urologist/her gyno states we have a great chance of it working 1st cycle. My wife is optimistic. We have scheduled our second appointment with a fertility specialist for the 2nd cycle if the first doesn't take. If IUI doesn't take on cycle 1 should we just move to IVF? When is (x number) of IUI's enough before giving up and moving to IVF?  How should I prepare should I release specimen before going to Dr. for the real specimen for a wash etc? any advice greatly apprciated.

What are your actual numbers? If you have a good count but a low motility IUI could improve your chances. When you do an IUI they 'wash' the sperm, basically spinning out all the non-motile sperm and leave you w/ just the good stuff.

Most REs like to do 3 cycles of any given protocol and then decide where to go from there. As long as you have a decent post wash number after IUI there is no reason why you couldn't continue doing that for a few cycles.

We usually have sex 2 days before we expect the IUI and then again the night of and the day after.



Me Dx: Weak Ovulation / Total Thyroid Removal
DH Dx: Mild MFI (borderline all 3), Varicocele
TTC#1 since 5.11
Clomid 50mg + TI = Poor Response
Clomid 100mg + TI (x2) = BFNs
Clomid 100mg+IUI#1 (5.6M Post Wash & 1 Follicle) = BFN
Surprise BFP on 1/20/13 EDD: 9/25/13 It's A Girl!!

I'd check with your doctor on this one. We have slightly low motility and one of her suggestions for improvement/better chances was to not BD more than once every 3 days except when ovulating. Though this recommendation might vary a great deal from one doc to another.

So you are doing an IUI with a OB and not a RE?  (or did I misunderstand?)


TTC #1 since July 2011.
BFP #1: 2/15/12-Ectopic/Mtx at 6 wks
BFP #2 10/12/12-m/c at 5 wks.
HSG all clear!! CD3 b/w normal. Urology appt all clear!
DH:MFI normal count (68mil) low motility (2%) and morph (9%)
Repeat SA: normal count (45.7mil), motility (47%) and low morph (1%), DNA frag: Excellent!
Me:MTHFR:Compound Heterozygous. On High Dose Folic Acid.
Cycle 17: Nov 2012: Clomid+ Trigger+ TI= BFN
Cycle 18: Dec 2012: Clomid+ Trigger+ TI+ progesterone= BFN
Cycle 19: Jan 2013: Clomid+ Trigger+ IUI #1+ TI+ progest =BFN
Cycle 20: Feb 2013: Clomid+ Trigger+ IUI #2+ TI+ progest =BFFN
Cycle 21: Mar 2013: Clomid+ Trigger+ IUI #3+ TI+ progest =BFN
Cycle 22: Apr 2013: Unmedicated TI+ progest =shocking BFN

IVF consult done. Waiting for CD 1 for testing, hysteroscopy, and BCP. Cycle in Aug or Sept. Anniversary

Sweets answered it best, so I am just going to add that it takes approx. 3 months for sperm to regenerate. Anything you do in the next 2 weeks will not help with your upcoming IUI.

Your doctor will tell you how long before the IUI you should ejaculate.

I am confused by your post because your doctor should have answered all these questions!

You also should be seeing a reproductive endocrinologist instead of a gynecologist for the IUI.

You probably shouldn't move to IVF after only trying IUI once. IVF costs a lot more money and is a LOT more invasive for your wife.

We are going to a fertility place that does the washing part of it then taking the specimen to her gyno to perform the procedure. My wife is more comfortable with her gyno. We set up appointment for next month if it doesn't take to have the fertility clinic do both procedures. 

 

What do you mean by BD? Thanks

BD=Baby Dance=Sex

Gynos don't specialize in getting you pregnant, and are not known for following the proper protocol for diagnosis and treatment.

Has your wife had an HSG and CD3 b/w and u/s?

I would not recommend treatment w/ a Gyno, I'd find an RE.

No it is not. GYNs do not get instruction in infertility. RE's are OB's that then study more years and do addition fellowships to earn that title. I will bet good money I know more than her GYN does and have spent more time researching IF issues.

You are wasting your money with her GYN. If you are made of money, then whatever. Waste away.

I'm curious- if you found out you had a cardiac condition, would you continue seeing your family doctor or would you consult a cardiologist?

I understand but her Gyno has fertility title maybe she isn't Gyno. We spoke to a few couples that she did iui and went well. We have an apt set up for feb for the fertility doc at the fertility clinic that is doing the washing for cycle 1. I appreciate the advice

We met with the fet dr. ran some tests and we have decided to skip the Gyno and use him. Hopefully next Friday will be the day.

I agree with this except it certainly doesn't hurt to start now because you never know how long it can take to conceive. So while starting some diet changes or some supplements now won't help the very next cycle, better to start now just in case.

read this link OP. Tons of great info! http://community.thebump.com/cs/ks/forums/thread/71182734.aspx One thing not mentioned in this link is walnuts! Apparently they are very good for you if you are dealing with MFI issues. My husband eats some everyday now. Good luck with the upcoming IUI. Glad you decided to go with the RE.

2nd SA Nov 2012 = 7.7 mil, Started Clomid for low count and low T.
March 2013: trigger + IUI#1 (4 Million) = BFN
April 2013: trigger + IUI#2 = Canceled :(
MRI of my uterus April 30th, result = Arcuate Uterus
May 2013: Femara + trigger + IUI#2 (2.7 Million) = BFP!!!

I have been eating a handful of walnuts since I found out I had low sperm count. Now the sperm count is very high motility is borderline low. Tuesday they take an ultrasound to check my wife's follicles since she has a lot which is good this way they can time iui better. I hope it takes on first cycle but we know it may take a few.
