MFI ? for a friend

A friend of mine just called me last night after RE called from husbands SA, it showed 800,000 sperm.  Any good websites or info I can pass along to her to help explain this?  This is her 2nd marriage and she has a DS already from her 1st so this is all new to her.  Since my DH and I are not MFI I am at a loss of what to say to her. 

Me (37) DH (39); Finally correctly diagnosed with DOR in 2012; Started TTC 2009; RE 2009/2010- RE cycle #1 clomid+ ovidrel + TI = BFN; RE cycle #2 clomid + ovidrel + TI=BFN; RE cycle #3 femara + ovidrel + IUI= BFN; RE cycle #4 75iu follistim + ovidrel + IUI= BFN; RE cycle #5 150iu follistim + ovidrel + IUI = BFN ; Took a break and had Weight loss surgery in 2011, back to new RE in 2012 ; AMH 0.43 IVF #1 /ICSI and AH; MDL protocol - ER 2/11 5R, 3M, 2F Three day transfer of 8 cell grade 3 and 8 cell grade 2 embryos. BFN. RPL testing shows homo MTHFR and hetero PAI-1. IVF #2 antagonist-ER 5/14 8R 6M 5F. Three day transfer of 3 embies. 6 cell grade 3, 4 cell grade 2, 5 cell grade 3. Hoping and praying. My dh originally had a low count. He takes a mens fertility vitamin the urologist we saw suggested coq10 so he takes that too. I would suggest repeating the test maybe seeing a urologist.

Me:28 DH:28 TTC since 1/11 Dx: unexplained IF/early DOR
Feb'12- Clomid/TI with CD3 &21 labs(normal)& SA-count 6mil/ml,morph/motil OK-BFN
April'12 Ultrasounds,HSG -clear, slight acurate uterus
5/14 IUI #1-50 mg clomid-post wash total motile count 19.3 prometrium 2dpiui= BFN
6/11-IUI #2-100 mg clomid & prometrium post wash count total motile 17.5 = BFN
7/6 IUI#3 with 100 mg clomid, estridiol, prometrium. post wash total motile count 23.3 = BFN
8/30IVF#1 BCPs, Follistim, Menopur,Ganerelix,Novarel.
ER 9/11-8R, 7M, 5F.
ET 9/14 2 embies transferred. 1 10cell Grade 4, 1 8cell Grade 4. No frosties. BFN
IVF#2 BCPs, Follistim, Menopur, Ganerelix, Novarel, Baby Aspirin
ER 12/5-16R, 12M, 8F!
ET 12/10 5dt! 1 fully expanded blast & 1 early blast. No frosties. BFN
2/13- Saline sono revealed a polyp. All additional labwork coming back normal. Genetic screen revealed DH has MTHFR deficiency & I am a carrier for it.
3/13 hysteroscopy & polypectomy
3/27 Began working with Reproductive Immunologist Dr. Kwak-Kim. Found High NK cells, High DHEAs, slight hypothyroidism, tendency toward blood clots (High PAI-1)Began synthroid & metformin 4/19.

Unfortantely, I don't know of any really good sites. We started with a reg Uro, which was a waste of time for us. If there is a Uro in her area that specializes in MFI, I would start there, but every treatment takes at least 3 months for updated results. Many REs don't try to improve the sperm, they just work with what you've got. DH was on Clomid (which didn't work for us) because morph, motility, and count were all way below normal. We only had 1.5 mil after wash, so IUI wasn't an option. Boxers and vitamins improved everything to normal except his morph, but we still needed IVF. Your friend should also ask about sperm DNA fragmentation. They think we might have that because 11 of our embryos/blasts arrested after day 3. GL to her. It's so frustrating there aren't many effective treatments for men.

OP is correct.  An MFI Uro is the way to go.  He will need to have labs drawn to check his hormone levels.  A regular uro was a complete waste of time.  It definitely sounds like they are IVF bound with that low number :(  Trying the supplements has worked for some people but not at all for us.  But I think its worth a try.  Can't hurt, might help.  

Me; 35 DH 32 TTC for over 2 years First mini-IVF Sept 2011... Only 1 egg! ... BFN Switching RE IVF#2 May 2012 9 eggs and only 2 sperm, WTH! BFN IVF#3 Chemical... FML. 2 frosties. SAIF/PAIF ALWAYS WELCOME!!!!!

I agree with everyone else...URL that specializes in MFI is the way to go! That being said, DH was told to take CoQ10 and they recommended a male supplement from Coast Science. It's a combo supplement. He was previously on clomid which improved his numbers somewhat, but current URL made him stop all but the supplements and this last SA was one of the better ones we have seen in a bit. Not great for things like IUI, but definitely good enough for IVF/ICSI. 

I know things like hcg and Follistim are Lso used to increase sperm counts, but starting that low, it certainly looks like IVF might be a real possibility
