3 sacs on ultrasound.

Today I went for my first prenatal appointment and now I am so, so confused.

Basically, the baby wasn't measuring where it should be for 8 weeks 4 days. It's measuring more like 5-ish weeks and has a heart rate of 80-86 BPM. The doctor said she saw everything she wanted to see, but it should be a lot further along if I ovulated when I think I did. Oh, and there was also three gestational sacks present. No fetal poles were visible yet, but she said it's still too early if I am 5 weeks. So, there's pretty much 3 different scenarios of what could be going on...

1) I had a weird cycle and I ovulated much later than I usually do, so I'm only 5 weeks along rather than 8 weeks along like my LMP suggests.

2.) I really am 8w4d along and it's not a viable pregnancy.

3.) I am 5 weeks along with multiples, which would explain me getting several positive pregnancy tests so early on due to elevated HCg levels.

She did a full blood panel (should get results tomorrow) and scheduled me to come back in 2 weeks. Basically I either have a baby that isn't growing right, a baby that's growing fine and just isn't as far along as I thought, or 2-3 babies.

Has anyone else ever experienced anything like this?  My head is spinning with all these crazy possibilities and I am trying to remain positive, but it's hard.

When did you get your first BFP?

My LMP was December 21st and I got my first positive on January 19th, two days before my period was due.

If your 1st bfp was jan 19th, the earliest you probably could have been would be about 7dpo if we assume you ovulated a bit later. Even that early, you should be further along than 5 weeks :/ hopefully the scan just wasnt able to pick up a good measurement they can be off especially early when everything is still so tiny. You saw a heartbeat which is always positive news. Hopefully it all works out, just start preparing yourself for the possibility that it might not. Thoughts prayers and hugs your way.

So there were three sacs and only one showed a baby with a heartbeat? I was just confused about "no fetal poles."

I'm confused about this too.  If there is no fetal pole, there wouldn't be a heartbeat.

They saw a fetal pole well before a hb, so that really confuses me. Sis they maybe get YOUR heartbeat instead and thought it was the baby's?

I wish you luck! I 'm also surprises they don't want to see you sooner to see how your HCG levels are doubling.

Honestly, I really don't know. This is my first pregnancy and the first time I've ever even heard of a fetal pole. I asked if the heartbeat she was picking up was mine, and she said it was not. There was clearly movement in 2 of the 3 sacs, so I'm not sure if that's what she was saying was a heartbeat? I was in such shock that I failed to ask a lot of the things I am wondering now. From what I understood, she said it was too early to see a fetal pole and that seeing empty sacs at this point can be normal.

It was clear she had never seen anything like it before. As we were checking out, she showed the ultrasound with 3 sacs to another Dr. and said, "ever done triplets before?"

So, I have no clue what to think. I'm calling for my blood results this morning and will ask about returning to see if HCG is doubling. Thank you girls!

It's hard to remember to ask questions or which ones to ask, so don't be hard on yourself! I would be kind of bewildered too if it was my first!

Glad you are calling back- if you need to sit down and write out questions before you call or go in, that might help clear your mind.

I am surprised they didnt want you back in a week. I had very long cycles and thought (based on online crap that wont let you change anything other than your LMP date I shouldve been 8w4d when I saw my OB, turns out I was 6w4d with twins that were clearly visable. I'd call back and ask to be seen sooner and why they didnt want repeat blood draws 2-3 days after the first to see if youre doubling normally.

ETA: high HCG levels isnt really a sign of multiples.

Now that everyone is saying it, I am surprised they don't want to see me back too. A friend of mine suggested I make an appointment with a doctor who specializes in multiples to get a second opinion, since it's obvious this doctor hasn't dealt with something like this before.

I know the blood draw won't tell me much and that high HCG isn't necessarily indicative of multiples. And low HCG could mean it's not doubling like it should or that I really am just 5 weeks. Two more weeks of waiting without knowing anything either way sounds completely torturous.

Give your current office a shot - ask about why  they are not taking repeated betas and have them answer some of your questions. If they do not assist or blow you off- get a second opinion from any OBGYN.

You will more than likely be referred to Meternal Fetal Medicine. They are Obs with a higher knowledge (for lack of a better word) of high risk pregnancies and multiples. I am lucky that my OB deals with this but if things get too risky he has no problem consulting or sending me to MFM.

I would just call. Ask for the reading on your U/S and for a referral to MFM or you can just go seek out new OB care on your own and see what they say.

There is a chance that there where multiples and the other two did not develope. That is what happened to me. I had triplets but one did not develope so it ended up just being twins. It actually happens a lot but is usually called the vanishing twin.

You said you saw movement clearly in 2 of the sacs. For there to be movement it had to be fetal cardiac activity so im not sure whats going on..... maybe you can call to get a full copy of your u/s report. They have to release it to you. At the very least request a thorough explanation of the appointment. You pay for those services and it is your body and your info, you are entitled to it.

My baby was measuring small too.  The dr. actually thought that it was a miscarriage or ectopic and even had me pick up the shot to clear everything out and that was at 6 weeks.  On a Thursday there was nothing to be seen during the internal ultrasound and then Monday presto - little baby with a heart beat.  It is always measuring a week behind.  And I had IVF so they know exactly when all the magic happened.  My obgyn said that sometimes the babies curl up a little and it makes them measure smaller.  But if you heard the heartbeat that is a great sign.  Sometimes they just take their own time growing I think and drs want everything to fit neatly on the chart.  Just call back and ask for another appointment and bring someone with you so that they will help you remember what the dr. said.  I had to bring my mom to all the bad, losing the baby, appointments because dh was oot for work and thank goodness because I did not ask anything or remember anything.  Good Luck!

I just got a call back from the doctors office. My HCG level is off the charts high, especially for how small the sacs are measuring.

My OB put a "stat" order in for me to get a more in-depth ultrasound at my local hospital possibly tomorrow or Friday. I know high HCG levels can mean a lot of things. I asked the nurse if I should be panicked, and she said no. That their equipment just isn't as sensitive as the hospitals, and that it's very likely that we are dealing with more than one baby.

So, once again...head spinning! I will be sure to keep you all updated after the ultrasound. The husband and I are going to sit down tonight and compile a list of questions we want to ask, since we both seem to go blank in the moment.

I really appreciate all of your help and suggestions.

Glad you got some answers!!! Don't put too much stock in your levels.  I had very high HcG levels at 4 weeks and only am having one baby =)
